Persistent Testicular Enlargement After Hernia Surgery: What to Do? - Urology

One year after hernia surgery, the testicles remain the same size?

Hello Doctor, I underwent laparoscopic surgery for a right inguinal hernia in March of last year.
The day after the surgery, I noticed that my left testicle had become enlarged and swollen (about twice the size of the right one).
At that time, the doctor said that this condition post-surgery was normal and that it would subside.
However, nearly a month later, the left testicle remained the same size, and there was no pain (it was only slightly uncomfortable when squeezed).
I then had an ultrasound and blood tests, which ruled out the possibility of testicular cancer.
The doctor also advised me not to worry.
Now, over a year later, although I have regained some sensation in the testicle, it is still the same size.
During this time, I have not experienced any significant discomfort.
Should I consider having further examinations for this situation?

Hello: Postoperative issues often involve some swelling.
If there are still problems after more than a year, it is advisable to return for a check-up.
Wishing you good health.

Persistent testicular enlargement following hernia surgery can be concerning, and it’s understandable that you’re seeking clarity on this issue. Based on your description, it seems that you underwent laparoscopic surgery for a right inguinal hernia, and shortly after the procedure, you noticed significant enlargement of the left testicle. While your doctor initially reassured you that this could be a normal post-operative occurrence, the fact that the swelling has persisted for over a year warrants further consideration.

Firstly, it’s important to understand that post-surgical changes can occur due to various reasons. Inguinal hernia repairs can sometimes lead to fluid accumulation in the scrotum, known as a hydrocele, or other conditions such as a varicocele, which is an enlargement of the veins within the scrotum. These conditions can cause the testicle to appear larger without necessarily indicating a serious underlying problem, especially if there is no pain or discomfort associated with the enlargement.

Since you have already undergone ultrasound imaging and blood tests that ruled out testicular cancer, this is a positive sign. However, the persistent nature of the enlargement suggests that it may be beneficial to have a follow-up evaluation. Here are some steps you might consider:
1. Follow-Up Consultation: It would be wise to consult with a urologist, who specializes in male reproductive health. They can perform a thorough examination and may recommend additional imaging studies if necessary to assess the nature of the enlargement.

2. Monitoring Symptoms: Keep track of any changes in symptoms. If you start to experience pain, discomfort, or any other unusual symptoms, it’s crucial to seek medical attention promptly.

3. Consideration of Surgical Intervention: If the enlargement is due to a hydrocele or varicocele and is causing discomfort or other issues, surgical intervention may be considered. This would typically involve a straightforward procedure to correct the issue.

4. Regular Check-Ups: Even if there are no immediate concerns, regular check-ups with your healthcare provider can help monitor the situation and ensure that any changes are addressed promptly.

5. Educate Yourself: Understanding the potential causes of testicular enlargement can help alleviate some anxiety. Conditions like hydrocele and varicocele are generally benign, but knowing more about them can empower you to make informed decisions about your health.

In conclusion, while the initial reassurance from your doctor is valuable, the persistence of the testicular enlargement over a year suggests that further evaluation is warranted. Engaging with a specialist can provide you with peace of mind and ensure that any necessary interventions are made in a timely manner. Remember, your health and comfort are paramount, and it’s always better to err on the side of caution when it comes to changes in your body.
