Prepuce and glans issues

Hello doctor! 1.
When not erect, the glans is hidden under the foreskin, and during an erection, the glans only protrudes slightly.
However, I can push it down to the coronal sulcus with my hand.
Do I still need a circumcision? 2.
Recently, I noticed a small red patch on the glans, and upon closer inspection, there are horizontal lines around it, and the coronal sulcus appears dark gray.
Is this normal? I masturbate about once a day; could this be the reason?

Lin, 20~29 year old female. Ask Date: 2017/11/12

Dr. Du Shixi reply Urology

Hello: 1.
Medically, it is not necessary; just keep the area clean and dry.
2.
If there has been unsafe sexual behavior, seek medical attention; if not, just be mindful of maintaining cleanliness.
Wishing you good health.

When it comes to understanding issues related to the foreskin and glans, particularly in the context of considering circumcision, there are several factors to take into account. Your questions touch on two main concerns: the retractability of the foreskin and the appearance of the glans.

1. Foreskin Retractability: You mentioned that when not erect, the glans is covered by the foreskin, and during an erection, it only partially exposes the glans. The ability to push the foreskin back to the corona (the ridge of the glans) is a positive sign, indicating that you have some degree of retractability. In general, if the foreskin can be retracted without pain and does not cause any discomfort or issues during sexual activity, circumcision may not be necessary. However, if you experience pain, difficulty retracting the foreskin, or if it causes hygiene issues, these could be valid reasons to consider circumcision.

2. Appearance of the Glans: The presence of a red patch on the glans and the horizontal lines you described could be indicative of several conditions. The discoloration and texture changes may be due to friction, irritation, or even a mild infection. It is not uncommon for individuals who masturbate frequently to experience some irritation, especially if adequate lubrication is not used. The dark gray color around the corona could be a normal variation in skin pigmentation, but it could also indicate irritation or a condition that requires further evaluation.

When to Consider Circumcision: Circumcision is typically considered in the following scenarios:
- Phimosis: This is a condition where the foreskin cannot be retracted fully over the glans. If you are unable to retract the foreskin comfortably, or if it causes pain or difficulty during erections, circumcision may be recommended.

- Recurrent Infections: If you experience frequent urinary tract infections or balanitis (inflammation of the glans), circumcision might be suggested as a preventive measure.

- Hygiene Issues: If you find it difficult to maintain proper hygiene due to the foreskin, circumcision can simplify this process.

- Pain or Discomfort: If the foreskin causes pain during sexual activity or erections, circumcision may be a viable option.

- Aesthetic or Personal Preference: Some individuals choose circumcision for personal, cultural, or religious reasons.

In your case, if the redness and markings on the glans persist or worsen, it would be advisable to consult a healthcare professional. They can provide a thorough examination and determine whether any treatment is necessary. It’s essential to avoid self-diagnosing and to seek professional advice, especially if there are changes in your body that concern you.

Lastly, while masturbation is a normal activity, if you notice that it leads to irritation or discomfort, consider adjusting your technique or frequency. Using lubrication can help reduce friction and prevent irritation.

In summary, circumcision is a personal decision that should be made based on medical advice, personal comfort, and hygiene considerations. If you have ongoing concerns about the appearance or health of your glans and foreskin, seeking a consultation with a urologist or a healthcare provider specializing in sexual health would be beneficial. They can provide tailored advice and address any underlying issues you may be experiencing.
