There is blood when wiping after a bowel movement?

Hello, doctor.
In the past few days, whenever I have a larger bowel movement, I notice fresh blood when wiping, and it feels a bit stingy.
After I finish, there is also a stinging, warm sensation, but there is no visible blood in the toilet or on the stool, only on the toilet paper.
My stool is quite thick and is a coffee or brownish-yellow color.
I do not have any abdominal pain.
What could this be?

Xiao Wen, 10~19 year old female. Ask Date: 2017/11/13

Dr. Ke Fangxu reply Surgery

Based on your description, it seems more like the typical symptoms of an anal fissure, which include pain during bowel movements accompanied by slight bleeding.
The common causes are often constipation or diarrhea.
The fundamental treatment is to improve bowel habits, along with warm sitz baths.
You can also purchase hemorrhoid suppositories at the pharmacy for relief.
If the symptoms persist without improvement, it is advisable to consult a physician for further evaluation.

More Info

Experiencing blood on toilet paper after bowel movements can be concerning, and it is essential to understand the potential causes and implications of this symptom. Based on the information provided, it appears that the blood is associated with the act of wiping after a bowel movement, particularly when the stool is hard or larger than usual. This scenario suggests a few possible explanations.

One of the most common causes of blood on toilet paper after a bowel movement is hemorrhoids. Hemorrhoids are swollen veins in the lower rectum or anus that can become inflamed and bleed, especially when there is straining during bowel movements or when passing hard stools. The blood is typically bright red, indicating that it is fresh and likely originates from the anal area rather than deeper within the gastrointestinal tract. The sensation of pain or a "stinging" feeling after wiping can also be indicative of hemorrhoids, as the sensitive tissue around the anus may be irritated.

Another possibility is anal fissures, which are small tears in the lining of the anus that can occur due to the passage of hard or large stools. Similar to hemorrhoids, fissures can cause sharp pain during and after bowel movements, and they may also lead to bleeding. The blood seen on toilet paper is usually bright red and may be accompanied by a burning sensation.

In your case, the description of the stool being hard and larger than usual, along with the absence of abdominal pain, further supports the likelihood of hemorrhoids or anal fissures. The fact that you mentioned the stool color as coffee or dirt-colored is generally normal, as stool color can vary based on diet and hydration levels. However, if you notice any significant changes in stool color, consistency, or if you experience other symptoms such as abdominal pain, weight loss, or changes in bowel habits, it is crucial to seek medical attention.

It is also worth noting that dietary factors can play a role in stool consistency. A diet low in fiber can lead to harder stools, which may increase the risk of developing hemorrhoids or fissures. Incorporating more fiber-rich foods, such as fruits, vegetables, and whole grains, can help soften stools and promote regular bowel movements. Staying well-hydrated is also essential for maintaining healthy bowel function.

While the symptoms you describe may be benign and related to hemorrhoids or fissures, it is essential to consult with a healthcare provider for a proper evaluation. They may recommend a physical examination to assess the anal area and rule out other potential causes of rectal bleeding, such as diverticular disease, colorectal polyps, or more serious conditions like colorectal cancer, especially if there are any concerning symptoms or a family history of gastrointestinal diseases.

In summary, blood on toilet paper after bowel movements is often due to hemorrhoids or anal fissures, particularly when associated with hard stools. However, it is vital to seek medical advice to ensure an accurate diagnosis and appropriate management. Making dietary adjustments to increase fiber intake and staying hydrated can help prevent future occurrences. If symptoms persist or worsen, do not hesitate to reach out to a healthcare professional for further evaluation.
