When is the Best Time for Scar Treatment After Injury? - Plastic Surgery

Scars that make me feel very insecure and lose confidence~ I kindly ask for the doctor's advice (includes images)?

When is the most appropriate time to undergo cosmetic treatment after a scar has formed? I've heard that treatments can be done one to two months after the wound has healed, while others say it's best to wait until the scar has matured (approximately six months to a year, or even longer for individuals with special conditions).
There are many opinions on this matter; which one is correct? Below is my scar: https://drive.google.com/open?id=1VpsnNQE0ZBlV3jqzdL8O6MHn7DQW-Y_y.
The left side shows the condition right after suturing in September, and the right side shows the appearance two months later in November.
It looks like a depressed scar.
What type of cosmetic treatment do you recommend for this scar? (Laser treatment? Scar revision surgery? Dermabrasion? Other options?) How effective are these treatments? I'm concerned about spending a lot of money without seeing significant results.
Since this scar is on my face, it is very noticeable, and I feel a lack of confidence and am quite depressed.
Please advise, thank you.

Shiluoren, 20~29 year old female. Ask Date: 2017/11/16

Dr. Lin Zhemin reply Plastic Surgery

Approximately three months after wound healing, aesthetic treatments such as laser resurfacing can be considered to improve scars.
Currently, re-excision and suturing of the scars is not recommended.

When it comes to scar treatment after an injury, timing is crucial for achieving the best results. The general consensus in the medical community is that the optimal time to start scar treatment varies based on the type of scar, the individual's healing process, and the specific treatment being considered.

Understanding Scar Formation:
After an injury, the body goes through a natural healing process that includes several phases: hemostasis, inflammation, proliferation, and remodeling. Scars typically begin to form during the proliferation phase and can take several months to mature. Initially, scars may appear red, raised, or discolored, but over time, they usually flatten and fade. This maturation process can take anywhere from a few months to a year or more, depending on factors such as the depth of the injury, the individual's skin type, and their overall health.

When to Start Treatment:
1. Immediate Post-Injury Phase (0-2 months): During this time, the focus should be on proper wound care to minimize scarring. Treatments such as silicone gel sheets or ointments can be beneficial in this phase to help keep the scar hydrated and reduce the risk of excessive scarring.

2. Early Scar Treatment (2-6 months): Many practitioners recommend starting treatments like silicone gel applications, pressure therapy, or even laser treatments within the first few months after the scar has formed. This is often when the scar is still malleable and responsive to treatment. However, it is essential to wait until the wound has fully healed and there are no open areas.

3. Mature Scar Treatment (6 months to 1 year): For scars that have matured, treatments such as laser therapy, chemical peels, or microneedling can be effective. At this stage, the scar is less likely to change significantly, and treatments can help improve its appearance. Some patients may also consider surgical options like scar revision if the scar is particularly bothersome.

4. Long-Term Considerations (1 year and beyond): For some individuals, especially those with specific skin types or conditions that predispose them to keloids or hypertrophic scars, it may take longer for scars to mature. In such cases, ongoing treatment may be necessary, and options like steroid injections or more advanced laser treatments can be explored.

Choosing the Right Treatment:
The choice of treatment depends on the scar's characteristics and the individual's preferences. Here are some common options:
- Laser Therapy: Effective for reducing redness and improving texture. Different types of lasers can target various aspects of scar appearance.

- Microneedling: Promotes collagen production and can improve the texture of scars.

- Chemical Peels: Help to exfoliate the top layer of skin, which can improve the appearance of superficial scars.

- Surgical Revision: In cases of significant scarring, surgical options may be considered to remove the scar tissue and re-close the wound.

Effectiveness and Expectations:
It's important to have realistic expectations regarding the effectiveness of scar treatments. While many treatments can significantly improve the appearance of scars, complete removal is often not possible. Consulting with a board-certified dermatologist or plastic surgeon specializing in scar management can provide personalized recommendations based on your specific situation.

In conclusion, the best time for scar treatment is typically after the wound has healed, within the first few months post-injury, but before the scar has fully matured. Each case is unique, so it's essential to consult with a medical professional who can assess your scar and recommend the most appropriate treatment options tailored to your needs.
