Pregnancy Uncertainty: Who is the Father? A Gynecological Dilemma - Obstetrics and Gynecology

Pregnancy without knowing the father?

Hello, my period started on October 1st and ended on the 6th.
Throughout October, my boyfriend and I had unprotected intercourse with ejaculation.
However, on the 16th, I had sexual intercourse with another man (let's call him Man B), and we used a condom the entire time.
After finishing, we took a short break (I wiped off the semen) and then continued for a second round.
We were still using a condom, but since the time between the two rounds was short, the second time was not as firm, so I removed the condom.
I went in a few times (but it was quite deep) and then stopped.
There was no ejaculation during the second round.
On the 28th, I went to the emergency room and found out I was pregnant, but my hCG levels were low at 250, and they couldn't see the embryo.
Today, on November 17th, I went to see an obstetrician and heard the heartbeat; the doctor said I was about five weeks along.
I'm really scared that there’s a possibility that the baby could be Man B's.
What are the chances of that? I had unprotected intercourse with my boyfriend during both the fertile and dangerous periods, but I'm still worried about the timing.

Determining the paternity of a child can be a complex and emotionally charged issue, especially when there are multiple potential fathers involved. In your case, you have outlined a timeline of sexual encounters with both your boyfriend and another man (referred to as "B"). Given the details you've provided, let's break down the situation and explore the factors that can help clarify paternity.

Understanding the Timing of Ovulation and Fertilization
First, it's essential to understand the timing of ovulation in relation to your menstrual cycle. You mentioned that your last menstrual period started on October 1st and lasted until October 6th. Typically, ovulation occurs about 14 days before the start of your next period. If we assume a regular cycle, ovulation would likely occur around October 15th. This is the time when you are most fertile, and any unprotected sexual intercourse around this date carries a higher risk of conception.

You indicated that you had unprotected intercourse with your boyfriend throughout October and also had a sexual encounter with "B" on October 16th, where you used a condom but later had a brief encounter without one. The fact that you had unprotected sex with your boyfriend during your fertile window increases the likelihood that he could be the father. However, the encounter with "B" also poses a risk, especially since you mentioned that the condom was removed during the second encounter.

Pregnancy Testing and Hormone Levels
You mentioned that you received a positive pregnancy test on October 28th, with a hormone level of 250. This indicates that you were indeed pregnant at that time. The hormone levels in early pregnancy can vary significantly, but the presence of a heartbeat during your follow-up appointment suggests that the pregnancy is progressing normally.

Paternity Testing
Given the uncertainty surrounding the paternity of your child, the most definitive way to determine who the father is would be through paternity testing. This can be done through a non-invasive prenatal paternity test (NIPP) that analyzes fetal DNA present in the mother's blood. This test can be performed as early as the 10th week of pregnancy and is highly accurate.

Emotional and Ethical Considerations
It's important to consider the emotional implications of this situation. Paternity can have significant impacts on relationships, family dynamics, and the child's future. Open and honest communication with both potential fathers may be necessary, especially if you are considering a paternity test.

Conclusion
In summary, while the timing of your sexual encounters suggests that your boyfriend is more likely to be the father due to the unprotected intercourse during your fertile window, the encounter with "B" cannot be entirely ruled out. The most reliable way to determine paternity is through genetic testing. It is advisable to consult with a healthcare provider or a genetic counselor to discuss your options and the implications of paternity testing. Remember, this is a sensitive issue, and approaching it with care and consideration for all parties involved is crucial.
