Severe ear discomfort and pain?
I have a runny nose, and when I pinched my nose and blew, I heard a loud "pop" in my left ear, followed by pain.
It felt like something had burst, and now my left ear hurts a lot, and I keep hearing a "buzzing" sound.
Can this be treated? Will it last a lifetime? What could be the cause? Is it a ruptured eardrum? This happened last night, and I thought it would get better after sleeping, but the pain is keeping me awake.
The clinic isn't open yet, and I'm a bit worried that this might be a long-term issue.

Hello, the issues you mentioned may have the following two possibilities:
1) Otitis Media:
a) Acute Otitis Media: This often occurs alongside upper respiratory infections (such as colds) and sinusitis, and in severe cases, there may be ear discharge or pus.
b) Chronic Otitis Media with acute exacerbation: This usually occurs when there is a pre-existing perforation in the eardrum, which can become inflamed due to unclean water or improper ear cleaning.
Treatment options include using ear drops combined with oral medications (such as antibiotics and anti-inflammatory pain relievers) for mild cases; hospitalization is only necessary for severe cases.
If hearing is affected, it is advisable to seek medical attention promptly.
2) Temporomandibular Joint Disorder (TMJ): This condition affects the joint located in front of the ear and is often associated with ear pain.
It is commonly caused by poor bite habits and can be managed with medication and adjustments to biting habits.
If symptoms persist, it is recommended to consult an otolaryngologist.
Wishing you peace and good health.
Sincerely,
Dr.

Ear pain can be a distressing experience, and understanding its causes is crucial for effective management. Based on your description, it seems that you may be experiencing symptoms related to Eustachian tube dysfunction or possibly a middle ear issue, such as barotrauma or even a perforated eardrum.
When you pinched your nose and blew, you likely increased the pressure in your Eustachian tubes, which connect the middle ear to the back of the throat. This maneuver is often used to equalize pressure in the ears, especially during altitude changes, such as flying or diving. However, if the Eustachian tube is not functioning properly, this can lead to discomfort and pain. The "popping" or "crackling" sound you heard could indicate that the pressure in your middle ear was not equalized effectively, leading to pain and possibly fluid buildup.
The symptoms you describe—pain in the left ear, a sensation of something "breaking," and the persistent "babababa" sound—could suggest that there is fluid in the middle ear or even a perforation of the eardrum. A perforated eardrum can occur due to sudden changes in pressure, infections, or trauma. If the eardrum is indeed perforated, it can lead to pain, hearing changes, and increased susceptibility to infections.
It's important to note that while some cases of ear pain resolve on their own, others may require medical intervention. If the pain persists, worsens, or is accompanied by other symptoms such as fever, drainage from the ear, or significant hearing loss, it is essential to seek medical attention as soon as possible. An ear, nose, and throat (ENT) specialist can perform a thorough examination, which may include otoscopy to visualize the eardrum and assess for any perforations or fluid accumulation.
In terms of treatment, if a perforation is confirmed, the management may vary based on the size and cause of the perforation. Many small perforations heal on their own, but larger ones may require surgical intervention, such as tympanoplasty. If there is an infection, antibiotics may be prescribed. Pain management is also crucial, and over-the-counter pain relievers can help alleviate discomfort.
Regarding your concern about whether this condition will last a lifetime, it is difficult to predict without a proper diagnosis. Many ear conditions are treatable, and with appropriate care, you can expect improvement. However, chronic issues can arise if underlying problems, such as allergies or recurrent infections, are not addressed.
In summary, while your symptoms may be alarming, they are not uncommon. The best course of action is to consult with a healthcare professional as soon as possible to determine the exact cause of your ear pain and to receive appropriate treatment. In the meantime, avoid inserting anything into your ear, and try to keep your head elevated to reduce pressure. If you experience any new symptoms or if the pain becomes unbearable, seek emergency medical care.
