Post-operative issues following fistula surgery?

Hello, Director Hsu: I underwent a fistula surgery in Taipei on December 29.
After the anal packing was removed on the 30th, I have been experiencing a discharge of blood-tinged fluid, and today, the 31st, I still have this fluid seeping out at home (which has a foul odor).
Is this a normal occurrence? Additionally, although I haven't taken a sitz bath daily, I have been using slightly warm water to spray and clean the area for at least 5 to 10 minutes.
Is this acceptable?

Mr. Tang, 50~59 year old female. Ask Date: 2017/12/31

Dr. Xu Kaixi reply Surgery

The recovery of the wound after fistula surgery typically takes about one to two weeks.
During this period, some bleeding or discharge is normal.
It is not recommended to rinse the wound directly with warm water, as this can be more irritating than sitz baths and may not be beneficial for wound healing.

More Info

After undergoing fistula surgery, it is not uncommon to experience some drainage, including blood and fluid, from the surgical site. However, the presence of a foul odor and continuous drainage can be concerning and may indicate a potential complication, such as infection or improper healing of the surgical site.

In the immediate postoperative period, it is normal for some blood-tinged fluid to drain from the site as the body begins to heal. This drainage may be mixed with serous fluid, which is a clear or slightly yellow fluid that can also be present. However, if the drainage is persistent, particularly if it has a strong odor, this could suggest that there is an issue that needs to be addressed.

Here are some important considerations regarding your situation:
1. Normal vs. Abnormal Drainage: While some drainage is expected, especially in the first few days post-surgery, it should gradually decrease over time. If the drainage is excessive, has a foul smell, or is accompanied by other symptoms such as increased pain, fever, or swelling, it is essential to contact your surgeon or healthcare provider immediately. These could be signs of an infection or other complications.

2. Hygiene Practices: It is good that you are maintaining cleanliness at the surgical site. Using warm water to gently clean the area can help reduce the risk of infection. However, avoid using harsh soaps or scrubbing the area too vigorously, as this can irritate the skin and potentially worsen the situation. Instead, continue with gentle cleansing methods and pat the area dry with a clean towel.

3. Monitoring Symptoms: Keep an eye on any changes in your symptoms. If you notice an increase in drainage, a change in the color or odor of the fluid, or any new symptoms, it is crucial to seek medical advice. Your healthcare provider may want to evaluate the site to ensure that there are no complications, such as an abscess or a persistent fistula.

4. Follow-Up Appointments: Ensure that you attend any scheduled follow-up appointments with your surgeon. These visits are important for monitoring your recovery and addressing any concerns you may have. If you feel that your concerns are not being adequately addressed, do not hesitate to reach out for a second opinion.

5. Signs of Infection: Be aware of signs that may indicate an infection, such as increased redness, warmth, swelling around the surgical site, fever, or chills. If you experience any of these symptoms, it is crucial to seek medical attention promptly.

In summary, while some drainage is expected after fistula surgery, persistent drainage with a foul odor is concerning and warrants further evaluation. Maintaining good hygiene and monitoring your symptoms closely will aid in your recovery. Always feel empowered to communicate with your healthcare provider about any concerns you have during your healing process. Your health and comfort are paramount, and addressing any issues early can lead to better outcomes.

It is normal for there to be drainage from the wound after laparoscopic surgery. Keeping the area clean and taking sitz baths can be beneficial. Applying ointments may lead to bacterial accumulation and make it difficult to keep the wound dry, so it is not advisable at this time.

It is recommended to continue warm sitz baths and apply ointment. If there is any discharge, you can gently express it to keep the area clean. If the swelling is gradually improving, there is no need to worry. Typically, there is no immediate recurrence after the procedure.
