Regarding benign prostatic hyperplasia (BPH)?

Recently, during a urology examination, the doctor mentioned that I have mild prostate enlargement, measuring about 4 cm and 3 cm on both sides (as seen on ultrasound).
I used to masturbate once a day.
Can young men with prostate enlargement see improvement? What can I do to help my prostate return to normal? Thank you.

She Hu Xian Yi Wen, 20~29 year old female. Ask Date: 2018/01/05

Dr. Du Shixi reply Urology

Hello: It is uncommon for natural hypertrophy to decrease in size.
However, if it is due to a disease causing swelling, it will subside after treatment.
Regular exercise, sitz baths, and a light diet can all be beneficial for the prostate.
Wishing you good health.

More Info

Prostate enlargement, or benign prostatic hyperplasia (BPH), is commonly associated with older men, but it can also occur in younger individuals, albeit less frequently. The condition is characterized by an increase in the number of cells in the prostate gland, leading to its enlargement. This can result in various urinary symptoms, including difficulty urinating, increased frequency of urination, and a weak urine stream.

Causes of Prostate Enlargement in Young Men
1. Hormonal Changes: The prostate is sensitive to hormones, particularly testosterone and its derivative dihydrotestosterone (DHT). Changes in hormone levels, which can occur during puberty or due to other factors, may contribute to prostate enlargement.

2. Genetic Factors: Family history can play a role in the likelihood of developing prostate issues. If there is a history of prostate problems in your family, you may be at a higher risk.

3. Lifestyle Factors: Diet, physical activity, and overall health can influence prostate health. A diet high in saturated fats and low in fruits and vegetables may contribute to prostate issues. Additionally, a sedentary lifestyle can exacerbate symptoms.

4. Inflammation or Infection: Prostatitis, or inflammation of the prostate, can also lead to enlargement. This condition can be caused by bacterial infections or other inflammatory processes.

5. Overstimulation: Frequent sexual activity, including masturbation, may lead to temporary swelling or discomfort in the prostate. While this is usually not a cause for concern, excessive stimulation could potentially lead to inflammation.

Solutions and Management Strategies
1. Lifestyle Modifications:
- Diet: Incorporate a balanced diet rich in fruits, vegetables, and healthy fats. Foods high in antioxidants, such as berries and leafy greens, can support prostate health.

- Exercise: Regular physical activity can improve overall health and may help alleviate some urinary symptoms. Aim for at least 150 minutes of moderate aerobic activity each week.

- Hydration: Drink plenty of water throughout the day, but consider reducing fluid intake in the evening to minimize nighttime urination.

2. Medical Treatment:
- If your symptoms are bothersome, consult with a urologist. They may recommend medications such as alpha-blockers or 5-alpha-reductase inhibitors, which can help relax the muscles around the prostate or reduce its size, respectively.

- Regular monitoring through follow-up appointments and ultrasounds may be necessary to track the size of the prostate and any changes in symptoms.

3. Avoiding Irritants:
- Limit caffeine and alcohol intake, as these can irritate the bladder and worsen urinary symptoms.

- Avoid spicy foods and artificial sweeteners, which may also contribute to bladder irritation.

4. Stress Management:
- Stress can exacerbate urinary symptoms. Techniques such as mindfulness, yoga, or meditation can help manage stress levels.

5. Regular Check-ups:
- Regular visits to a healthcare provider for monitoring and evaluation are crucial, especially if you notice changes in urinary habits or experience discomfort.

Conclusion
While prostate enlargement in young men can be concerning, it is often manageable with lifestyle changes and medical intervention. It is essential to communicate openly with your healthcare provider about your symptoms and concerns. They can provide personalized advice and treatment options based on your specific situation. Remember, early intervention can lead to better outcomes, so don’t hesitate to seek help if you have ongoing issues.

It is recommended that you consult a urologist to rule out whether it is a hernia or other issues. Benign prostatic hyperplasia requires a digital rectal examination for diagnosis. Your age should not predispose you to this problem.
