Penile swelling?

Hello, doctor.
I masturbate 1 to 2 times a day.
A few days ago, while masturbating, I held back for a while and suddenly started feeling pain.
I realized that my phimosis had completely retracted due to the pain, and I stopped masturbating halfway.
After that, the middle part became swollen, and touching it still hurts.
The glans has not returned to its original position, and now phimosis has become much easier to retract.
Later, when I urinate, the urine sprays very far, and sometimes it splits into two streams—one side sprays far while the other side drips, making a mess everywhere.
Is this serious?

Niming, 20~29 year old female. Ask Date: 2018/01/08

Hello: It is not a mild condition; the foreskin needs to be retracted back to its original position.
If that is not possible, medical attention is necessary, as poor circulation may lead to increasing swelling.
Wishing you good health.

It sounds like you're experiencing some concerning symptoms following masturbation, particularly related to penile swelling and pain. Let's break down your situation and address your concerns.

Firstly, the swelling and pain you're experiencing could be attributed to a few different factors. One possibility is that the prolonged stimulation during masturbation may have caused some irritation or trauma to the penile tissues, particularly if you were in a position that put pressure on the area. This can lead to swelling, which might explain the discomfort and the difficulty in retracting the foreskin (if you are uncircumcised).
Another potential cause could be a condition known as paraphimosis, which occurs when the foreskin is retracted behind the glans (the head of the penis) and cannot be returned to its original position. This can cause swelling and pain and may require medical intervention to resolve. If you are unable to return the foreskin to its normal position, it is important to seek medical attention promptly, as this condition can lead to complications if left untreated.

Regarding the changes in urination, such as the urine spraying in different directions, this could be related to the swelling or changes in the anatomy of the urethra due to the swelling. When the urethra is compressed or distorted, it can lead to a split stream of urine. This is generally not a serious issue, but it can be uncomfortable and may indicate that you should have a healthcare professional evaluate your symptoms.

It's also worth noting that frequent masturbation, while generally considered safe, can sometimes lead to temporary discomfort or irritation if done excessively or without adequate lubrication. If you find that your symptoms persist or worsen, it would be wise to consult a healthcare provider. They can perform a physical examination and may recommend tests to rule out any infections or other underlying conditions.

In terms of severity, while your symptoms may be uncomfortable, they are not necessarily indicative of a serious medical condition. However, if you experience severe pain, fever, or any signs of infection (such as discharge or an unusual odor), you should seek medical attention immediately.

In conclusion, while your symptoms may be concerning, they are often manageable with proper care. If the swelling and pain do not improve or if you have any additional symptoms, please consult a healthcare professional for a thorough evaluation. They can provide you with the appropriate treatment and guidance to ensure your health and well-being. Remember, it's always better to err on the side of caution when it comes to your health.
