Numbness in the limbs and mild chest pain when exhaling have been experienced recently. I kindly request the physician's advice and assistance?

Hello, Doctor He.
Since July 2017, I have been experiencing numbness in my left foot.
I have visited both Yadong Hospital and Shuanghe Hospital, but they have not been able to determine the cause.
I have undergone X-rays (cervical and lumbar spine), nerve conduction studies, evoked potentials, and EEG tests.
As of January, the numbness has progressed from my left foot to all four limbs, and I have now been referred to Taipei Mackay Memorial Hospital for further evaluation.

The lack of improvement in my condition has made me very anxious, and I am concerned that I may have a serious illness.
I find myself worrying throughout the day while at work.
Recently, I have experienced mild chest pain when taking deep breaths, and during my last visit to the hospital, my blood pressure was measured at 130-145 with a pulse of 95-105, which is also elevated (I am a smoker).

I would like to ask the doctor which heart conditions might be related to my symptoms of limb numbness.
I feel helpless and frustrated every day; I experience palpitations during the day and have been waking up frequently in the early morning hours.
I am unsure what is happening to my body! Please, I kindly ask for your advice and assistance!

Hello, you have undergone almost all the necessary examinations and everything seems fine, so there is likely no cause for concern.
It appears that the symptoms you are experiencing may be due to anxiety-induced autonomic nervous system dysregulation.
It would be advisable to consult a neurologist or a psychiatrist.
Take care, and we at Changhua Hospital are here for you.

It sounds like you are experiencing a complex set of symptoms that are understandably causing you a great deal of concern. The combination of numbness in your limbs, chest pain, elevated heart rate, and anxiety can indeed be alarming, and it's important to approach these symptoms with a comprehensive understanding of potential underlying issues.

First, let's address the numbness in your limbs. Numbness can arise from a variety of causes, including neurological conditions, vascular issues, or even musculoskeletal problems. Given that you've undergone extensive testing, including X-rays, nerve conduction studies, and EEGs, it seems that significant neurological conditions have been ruled out. However, the progression of numbness from one limb to all four could suggest a systemic issue or a problem that is not easily identifiable through standard imaging or tests.

The chest pain you mentioned, especially when accompanied by shortness of breath or a feeling of pressure, can be concerning for heart-related issues. While your elevated blood pressure and heart rate could be indicative of anxiety or stress, they can also be signs of an underlying cardiovascular condition. Given your history of smoking, it is essential to consider the risk factors for coronary artery disease (CAD) or other heart conditions. Smoking is a significant risk factor for cardiovascular diseases, and it can contribute to the development of atherosclerosis, which can lead to chest pain and other symptoms.

Your symptoms of anxiety, including feeling overwhelmed and experiencing palpitations, can exacerbate physical symptoms, creating a cycle of worry and discomfort. Anxiety can manifest physically in many ways, including increased heart rate, chest tightness, and even numbness or tingling sensations. It is crucial to address both the physical and psychological aspects of your health.

In terms of potential heart-related concerns, the symptoms you describe could be associated with several conditions, including:
1. Coronary Artery Disease (CAD): This occurs when the coronary arteries become narrowed or blocked, leading to reduced blood flow to the heart muscle. Symptoms can include chest pain (angina), shortness of breath, and fatigue.

2. Anxiety Disorders: Anxiety can mimic heart disease symptoms, leading to chest pain, palpitations, and a feeling of impending doom. It is essential to differentiate between anxiety-induced symptoms and those caused by actual cardiac issues.

3. Peripheral Neuropathy: This condition affects the peripheral nerves and can lead to numbness and tingling in the limbs. While it may not be directly related to heart issues, it can coexist with other conditions.

4. Panic Attacks: These can cause sudden episodes of intense fear or discomfort, leading to physical symptoms such as chest pain, rapid heartbeat, and shortness of breath.

Given your symptoms and the potential overlap between anxiety and heart conditions, it is advisable to consult with a cardiologist for a thorough evaluation. This may include further testing such as an echocardiogram, stress testing, or even a Holter monitor to assess your heart's rhythm over a period of time. Additionally, a referral to a mental health professional could be beneficial to address the anxiety and stress you are experiencing.

In the meantime, consider lifestyle modifications that can improve both your physical and mental health. Quitting smoking, engaging in regular physical activity, and practicing relaxation techniques such as mindfulness or yoga can be beneficial. It's also essential to maintain a healthy diet and ensure you are getting adequate sleep.

Remember, you are not alone in this, and seeking help from healthcare professionals can provide you with the support and answers you need. Your health is a priority, and addressing both the physical and emotional aspects of your symptoms is crucial for your overall well-being.
