Chronic Nonbacterial Prostatitis

Hello, Doctor.
I have a few questions regarding chronic prostatitis.
Three months ago, I visited a urologist due to frequent urination, urinary retention, and a burning sensation in the urethra.
The doctor diagnosed me with chronic prostatitis, and the urinalysis showed no signs of inflammation.
After six weeks of medication (Fluoroquinolone), I felt healed, and the doctor suggested I could try stopping the medication.
However, after five days off the medication, the symptoms of prostatitis reappeared.
The doctor then prescribed Chemix tablets, and I have been taking them for another six weeks.
I feel that the medication is controlling the symptoms, but I still experience a burning sensation during urination.
Currently, I do not have frequent urination or urinary retention, but the burning sensation is quite bothersome.
I would like to ask the doctor a few questions:
1.
With the recurring symptoms of chronic prostatitis, is there a possibility of complete recovery? I am genuinely worried as it has affected my daily life.
2.
I have read online that "low-energy extracorporeal shock wave therapy" can improve this condition.
Is this treatment applicable only to patients with sexual dysfunction, or can it be used for all cases of prostatitis?
3.
Regarding antibiotics, since chronic prostatitis requires treatment for 4-12 weeks, is there a risk of developing antibiotic resistance with long-term use?
4.
Due to taking antibiotics, both good and bad bacteria in the gut are affected.
Should I consider taking probiotics to help restore balance? If so, when is the best time to take probiotics during the course of antibiotic treatment?
5.
If prostatitis recurs frequently, does this increase the risk of developing prostate cancer in middle-aged and older men?

Blesshin, 30~39 year old female. Ask Date: 2018/01/11

Dr. Sun Hongshun reply Urology

1.
Chronic prostatitis is termed "chronic" because it is difficult to cure and prone to recurrence.

2.
Low-energy extracorporeal shock wave therapy is very effective in treating chronic prostatitis, and I have many cases of patients who have experienced relief after treatment.

3.
There will be no issues with drug resistance when combined with treatment.

4.
There are no specific effects; if more details are needed, please consult a gastroenterologist.

5.
Currently, there are no research reports indicating a positive correlation.

Chronic prostatitis, particularly chronic pelvic pain syndrome (CPPS), can be a challenging condition to manage due to its complex nature and the variability of symptoms. Your concerns about the recurring symptoms and the impact on your daily life are valid, and it's important to address them comprehensively.

1. Will chronic prostatitis ever heal?
Chronic prostatitis is often characterized by its persistent and recurrent symptoms, which can make it feel like a never-ending battle. While some patients do experience significant improvement or even remission, others may find that symptoms persist or recur despite treatment. The key to managing chronic prostatitis lies in a multifaceted approach that includes medication, lifestyle modifications, and possibly alternative therapies. Regular follow-ups with your healthcare provider are essential to monitor your condition and adjust treatment as necessary.

2. Low-energy extracorporeal shock wave therapy (ESWT):
This treatment has gained attention for its potential benefits in managing chronic prostatitis symptoms. While it is primarily used for erectile dysfunction, studies suggest that it may also help alleviate pelvic pain associated with prostatitis. The therapy works by promoting blood flow and reducing inflammation in the prostate area. It is generally considered safe and may be an option for patients who do not respond well to conventional treatments. However, it's essential to discuss this option with your urologist to determine if it's suitable for your specific case.

3. Antibiotic resistance concerns:
Long-term antibiotic therapy can indeed raise concerns about antibiotic resistance, especially if antibiotics are used unnecessarily or for extended periods. In cases of chronic prostatitis where bacterial infection is not the primary cause, antibiotics may not be the most effective treatment. It's crucial to have a thorough evaluation to determine the underlying cause of your symptoms. If antibiotics are deemed necessary, your doctor will likely monitor your treatment closely to mitigate the risk of resistance.

4. Probiotics during antibiotic treatment:
Taking probiotics can be beneficial when you're on antibiotics, as they help restore the balance of good bacteria in your gut that antibiotics may disrupt. It's generally recommended to take probiotics a few hours after your antibiotic dose to maximize their effectiveness. For example, if you take your antibiotic in the morning, consider taking the probiotic later in the day. This timing helps ensure that the antibiotic does not kill the beneficial bacteria in the probiotic before they can provide their benefits.

5. Risk of prostate cancer with recurrent prostatitis:
Current research does not conclusively link chronic prostatitis to an increased risk of prostate cancer. However, chronic inflammation has been suggested as a potential risk factor for various cancers, including prostate cancer. It's essential to maintain regular check-ups with your healthcare provider, especially as you age, to monitor your prostate health. Discussing your concerns about prostate cancer with your doctor can help you understand your individual risk factors and the importance of screening.

In summary, managing chronic prostatitis requires a comprehensive approach that includes medication, lifestyle changes, and possibly alternative therapies. Regular communication with your healthcare provider is crucial to navigate this condition effectively. Don't hesitate to seek support from healthcare professionals who can provide guidance tailored to your specific situation. Remember, you're not alone in this journey, and there are resources available to help you manage your symptoms and improve your quality of life.
