Irregular menstruation

At first, my menstrual cycle was regular, but it started to become irregular when I was in the second year of junior high school.
Initially, it was just delayed, but eventually, I stopped getting my period altogether.
I went to a traditional Chinese medicine practitioner to adjust my body, but the medication took a long time, and I didn't see any improvement.
I then switched to Western medicine and took medication to induce my period, which worked, but as soon as I stopped taking it, my period would not come back.
My mom heard that Western medicine is bad for the body, so I went back to traditional Chinese medicine, and I kept switching back and forth.
However, it seems that whenever I stop taking the medication, my period just doesn't come.
After a while, I stopped getting the medication altogether, and my period ceased completely.

Today, I experienced severe abdominal pain and had to go to the bathroom.
Initially, I had diarrhea, but when I went again, I passed a large amount of blood.
What could be happening? Am I sick?

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors that can affect the menstrual cycle or cause intermenstrual bleeding are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraception, diet, polycystic ovary syndrome, uterine or cervical lesions, infections, ovulation, pregnancy, and breastfeeding, among others.
Typically, in females under the age of 18, ovarian function has not yet matured.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

Irregular periods can be a source of significant concern for many individuals, particularly when they experience drastic changes in their menstrual cycle. Your situation, where you have transitioned from having regular periods to experiencing delays and eventually missed periods, is not uncommon. Several factors can contribute to irregular menstrual cycles, and understanding these can help in determining the appropriate treatment options.

Causes of Irregular Periods
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones. Any disruption in this hormonal balance, such as fluctuations in estrogen and progesterone, can lead to irregular periods. Conditions like polycystic ovary syndrome (PCOS) are common culprits, characterized by hormonal imbalances that can cause missed periods, excessive hair growth, and weight gain.

2. Stress and Lifestyle Changes: Emotional and physical stress can significantly impact your menstrual cycle. Changes in your daily routine, such as moving to a new environment, changes in diet, or increased physical activity, can also lead to irregularities.

3. Diet and Weight Fluctuations: Significant weight loss or gain can affect your menstrual cycle. Being underweight or overweight can lead to hormonal changes that disrupt your cycle.

4. Medical Conditions: Certain medical conditions, such as thyroid disorders, diabetes, and uterine abnormalities, can also cause irregular periods. It is essential to rule out these conditions through appropriate medical evaluations.

5. Medications: Some medications, including hormonal contraceptives and certain psychiatric medications, can affect your menstrual cycle. If you have been switching between traditional Chinese medicine and Western medicine, the changes in medication can also contribute to irregularities.

Treatment Options
1. Lifestyle Modifications: Addressing lifestyle factors such as stress management, maintaining a healthy weight, and ensuring a balanced diet can help regulate your menstrual cycle. Regular exercise can also be beneficial.

2. Hormonal Treatments: If hormonal imbalances are identified as the cause of your irregular periods, hormonal treatments may be prescribed. This could include birth control pills or other hormonal therapies that help regulate your cycle.

3. Alternative Therapies: If you prefer traditional Chinese medicine, it may be worth discussing with a qualified practitioner who can provide a tailored approach to your condition. However, it is essential to ensure that any alternative treatments are safe and do not interfere with any ongoing medical treatments.

4. Monitoring and Evaluation: Keeping a menstrual diary can help track your cycle and identify patterns. This information can be valuable for your healthcare provider in determining the best course of action.

5. Consultation with a Specialist: If your irregular periods persist or if you experience severe symptoms, such as significant pain or heavy bleeding, it is crucial to consult with a gynecologist. They may recommend further evaluations, such as blood tests or imaging studies, to identify any underlying conditions.

Conclusion
Your experience of irregular periods and the recent episode of heavy bleeding is concerning and warrants further investigation. It is essential to seek medical attention to rule out any serious underlying conditions. A healthcare provider can help you navigate the complexities of your menstrual health and develop a personalized treatment plan that addresses your specific needs. Remember, it is crucial to communicate openly with your healthcare provider about your symptoms, treatment preferences, and any concerns you may have regarding the medications you are taking.
