Bleeding on the fourth day after wisdom tooth extraction?

I had my upper and lower right wisdom teeth extracted last Friday (January 19).
The lower one was horizontal, so I had to undergo surgery and stitches.
However, today (January 23), I suddenly noticed that the stitches from the wound seem to be protruding more than usual, and there has been continuous oozing of blood.
I'm hesitant to apply heat compresses for fear of causing a sudden increase in bleeding.
I've only been consuming milk with sponge cake, and my face is swollen, with the wound feeling somewhat tight.
Should I return to the doctor for a check-up regarding my condition?

Bleeding after wisdom tooth extraction is a common concern and can vary in intensity depending on several factors, including the complexity of the extraction, the patient's individual healing process, and any medications they may be taking. In your case, it seems that you had a surgical extraction of your lower wisdom teeth, which can lead to more significant bleeding compared to a simple extraction.
It is normal to experience some bleeding or oozing from the extraction site for the first few hours after the procedure. However, if bleeding persists beyond 24 hours or if it is heavy, it is essential to take appropriate measures. The fact that you are noticing an increase in the visibility of the sutures and ongoing oozing suggests that your body may be having difficulty forming a stable clot, or there may be other underlying issues at play.

Here are some steps you can take to manage the situation:
1. Bite on Gauze: If you haven't already, place a clean piece of gauze over the extraction site and bite down firmly for about 30 minutes. This pressure can help promote clot formation.

2. Avoid Spitting or Rinsing: For the first 24 hours, avoid spitting or rinsing your mouth, as these actions can dislodge the clot and lead to further bleeding.

3. Stay Upright: Keeping your head elevated can help reduce blood flow to the area and minimize bleeding.

4. Cold Compress: Applying a cold compress to the outside of your cheek can help reduce swelling and numb the area, which may alleviate some discomfort.

5. Avoid Straws and Hard Foods: Using straws can create suction that may dislodge the clot. Stick to soft foods and liquids for the time being.

6. Monitor Symptoms: Keep an eye on your symptoms. If the bleeding continues or worsens, or if you experience severe pain, swelling, or fever, it is crucial to contact your dentist or oral surgeon.

Given that you are experiencing ongoing bleeding and discomfort, it is advisable to return to your dentist for a follow-up appointment. They can assess the extraction site, check for any complications such as dry socket (a condition where the blood clot fails to form or is dislodged), and determine if any additional treatment is necessary.
In terms of your concern about the sutures, it is not uncommon for sutures to appear more pronounced as swelling occurs. However, if they seem excessively loose or if you have any concerns about the healing process, your dentist will be able to provide clarity and reassurance.

In summary, while some bleeding is normal after wisdom tooth extraction, persistent or heavy bleeding warrants a visit to your dentist. They will be able to evaluate your situation and provide the necessary care to ensure proper healing. Remember, it’s always better to err on the side of caution when it comes to your health, especially after a surgical procedure.
