Left abdominal pain
Hello, Doctor: For the past month, I have frequently experienced sharp pain in the upper left abdomen, occurring about 2 to 3 times a day.
The pain usually lasts only a few seconds and tends to happen after eating.
This has been ongoing for more than a month, and I am unsure what the issue might be (PS: I eat quite quickly).
Additionally, after having a bowel movement, I usually wipe multiple times until I feel clean.
However, recently, after sitting in the office for about 1 to 2 hours post-bowel movement, I start to feel itching around the anal area.
When I go to the restroom and wipe again, I notice that it seems I haven't wiped thoroughly.
This may be the cause of the itching; once I clean it properly, the itching sensation significantly decreases.
However, I am puzzled because I always check to ensure I am clean before stopping, yet this anal itching has been occurring almost daily, and I keep discovering that I haven't wiped properly.
Could you please advise on what might be causing these issues? These two problems have been quite bothersome for me.
Thank you, Doctor!

Hello, sir.
It is recommended to start with regular, small meals and to eat slowly to see if there is any improvement.
If necessary, you may visit a gastroenterology clinic for an examination.
Additionally, after using the restroom, it is advised to wash the anal area with water, dry it thoroughly, and keep it dry to reduce discomfort.
Changhua Hospital cares about your health.
Sincerely, Chen Shih-Dian.

More Info
Upper left abdominal pain can be a perplexing symptom, especially when accompanied by other issues such as anal itching. Based on your description, it appears that you are experiencing two distinct but potentially related problems: intermittent pain in the upper left abdomen and persistent anal itching after bowel movements.
Understanding Upper Left Abdominal Pain
The upper left abdomen houses several vital organs, including parts of the stomach, pancreas, spleen, and left kidney. Pain in this area can arise from various conditions, including:
1. Gastric Issues: Given that your pain occurs after eating, particularly after meals, it could be related to gastric problems such as gastritis or even peptic ulcers. Eating quickly can lead to overeating, which may exacerbate these conditions, causing discomfort or pain.
2. Pancreatitis: Inflammation of the pancreas can also cause pain in the upper left abdomen. This condition may be acute or chronic and is often associated with other symptoms like nausea or vomiting.
3. Splenic Issues: Conditions affecting the spleen, such as splenomegaly (enlargement of the spleen) or splenic infarction, can lead to pain in the upper left quadrant.
4. Musculoskeletal Pain: Sometimes, pain in the abdominal area can be due to muscle strain or other musculoskeletal issues, especially if you have engaged in physical activities that may have strained the abdominal muscles.
5. Gastroesophageal Reflux Disease (GERD): If you have a history of acid reflux, it could also manifest as pain in the upper abdomen, particularly after meals.
Anal Itching
The anal itching you are experiencing can be attributed to several factors:
1. Hygiene Issues: As you mentioned, you feel the need to wipe excessively to ensure cleanliness. Over-wiping can irritate the sensitive skin around the anus, leading to itching. Conversely, inadequate cleaning can also cause irritation and itching.
2. Hemorrhoids: Swollen veins in the rectal area can cause itching and discomfort, especially after bowel movements. They can be exacerbated by straining during bowel movements or prolonged sitting.
3. Skin Conditions: Conditions such as eczema or psoriasis can affect the anal area, leading to itching.
4. Dietary Factors: Certain foods can irritate the digestive tract and lead to itching. Spicy foods, caffeine, and alcohol are common culprits.
5. Infections: Fungal infections or pinworms, particularly in children, can cause anal itching.
Recommendations
1. Dietary Modifications: Consider slowing down your eating pace and being mindful of portion sizes. Eating smaller, more frequent meals may help alleviate post-meal pain. Additionally, avoid spicy, fatty, or overly processed foods that may irritate your stomach.
2. Hydration and Fiber: Ensure you are drinking enough water and consuming a diet rich in fiber to promote healthy bowel movements, which may help reduce anal itching and discomfort.
3. Hygiene Practices: After bowel movements, consider using moist wipes instead of dry toilet paper to ensure cleanliness without irritation. Avoid excessive wiping, as this can lead to skin irritation.
4. Consult a Healthcare Provider: Given that your symptoms have persisted for over a month, it is crucial to consult a healthcare professional. They may recommend diagnostic tests such as an ultrasound, endoscopy, or stool tests to rule out any underlying conditions.
5. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated factors (like diet or activities). This information can be invaluable for your healthcare provider in diagnosing the issue.
In conclusion, while your symptoms may be benign, they warrant further investigation to rule out any serious underlying conditions. A thorough evaluation by a healthcare professional is essential for accurate diagnosis and effective treatment.
