Urinary dysfunction

Hello, Doctor.
I am 24 years old and have been experiencing this condition for about four months.
I usually drink little water, and since the onset of frequent urination, I am even more hesitant to drink.
I urinate approximately every 30 minutes to an hour, and I experience itching at the urethral opening.
The urine is clear, but I often feel like there is residual urine after urination.
I have a strong urge to urinate, and the odor is quite foul (fishy).
Previously, I did not have these symptoms, but sometimes after urination, I feel a sharp pain at the base of the penis.
I have consulted a doctor who suggested it might be chronic cystitis or hyperactivity and prescribed medication (Urorec).
After taking it for about a month, there hasn't been much improvement.
I would like to ask if there is a definitive treatment method or if I should undergo a prostate examination?

Dr. Du Shixi reply Urology

Hello: You need to see a urologist for an examination; just taking medication may not be effective.
Wishing you good health.
Currently, avoid drinking beverages and spicy or irritating foods.

Urinary disorders can significantly impact a person's quality of life, and understanding their symptoms, causes, and treatment options is crucial for effective management. Based on your description, it appears that you are experiencing a range of urinary symptoms, including frequent urination, urgency, a strong odor in your urine, and discomfort during urination. These symptoms can be indicative of several underlying conditions, including chronic cystitis (bladder inflammation), urinary tract infections (UTIs), or even issues related to the prostate, especially in males.

Symptoms of Urinary Disorders
1. Frequent Urination: Needing to urinate more often than usual, which can be distressing and inconvenient.

2. Urgency: A sudden, compelling urge to urinate that may be difficult to control.

3. Dysuria: Pain or discomfort during urination, which can be a sign of infection or inflammation.

4. Hematuria: Blood in the urine, which should always be evaluated by a healthcare provider.

5. Residual Urine: The sensation of not fully emptying the bladder, which can lead to further complications.

6. Odor Changes: A strong or foul-smelling urine can indicate infection or other metabolic issues.

Possible Causes
1. Infections: UTIs are common causes of urinary symptoms, especially in young adults. They can lead to inflammation and discomfort.

2. Chronic Cystitis: This condition involves long-term inflammation of the bladder, which can be caused by recurrent infections or irritants.

3. Prostate Issues: In males, prostate enlargement or inflammation can lead to urinary difficulties, including urgency and incomplete emptying.

4. Neurological Conditions: Conditions affecting nerve function can disrupt normal bladder control.

5. Medications: Some medications can affect bladder function and urinary patterns.

Treatment Options
1. Antibiotics: If a UTI is confirmed, antibiotics are typically the first line of treatment. It's essential to complete the full course as prescribed.

2. Anti-inflammatory Medications: These can help reduce inflammation in the bladder and alleviate symptoms.

3. Bladder Training: This involves scheduled urination to help retrain the bladder and reduce urgency.

4. Medications for Overactive Bladder: Anticholinergic medications can help manage symptoms of urgency and frequency.

5. Dietary Changes: Avoiding irritants such as caffeine, alcohol, and spicy foods may help reduce symptoms.

6. Pelvic Floor Exercises: Strengthening the pelvic floor muscles can improve bladder control.

7. Further Evaluation: If symptoms persist despite treatment, further evaluation, including imaging studies or cystoscopy, may be necessary to rule out structural issues or other underlying conditions.

Recommendations
Given your symptoms and the lack of improvement with the current medication, it may be beneficial to revisit your healthcare provider for a comprehensive evaluation. This could include:
- Urinalysis and Culture: To check for infections or abnormalities.

- Imaging Studies: Such as an ultrasound to assess the bladder and kidneys.

- Cystoscopy: A procedure that allows direct visualization of the bladder and urethra, which can help identify any abnormalities or sources of inflammation.

- Prostate Evaluation: If you are male, discussing prostate health with your doctor may be warranted, especially if you are experiencing pain at the base of the penis.

In conclusion, urinary disorders can stem from various causes, and a tailored approach to diagnosis and treatment is essential. Open communication with your healthcare provider about your symptoms and concerns will facilitate a more effective management plan. Remember, early intervention can lead to better outcomes and improved quality of life.
