Major bleeding

Hello, Director Liao: This morning, after having a bowel movement, I noticed a small amount of bright red blood on the toilet paper.
I pressed a bit harder, which resulted in more blood.
By the afternoon, when I had another bowel movement, I found blood mixed in the toilet water, and there was even more blood on the toilet paper compared to this morning.
I experienced a similar situation a few months ago.
Could you please explain what this condition might be? Additionally, which specialty should I consult for this issue? Thank you.

Buffert, 30~39 year old female. Ask Date: 2018/02/04

Hello, it is recommended to consult a colorectal specialist for evaluation to determine if an endoscopy is necessary to clarify the possible causes of bloody stools.
Please note that online consultations do not equate to in-person medical visits and cannot provide accurate diagnoses or treatment recommendations.

Rectal bleeding can be a concerning symptom, and understanding when to seek medical attention is crucial for your health. Based on your description, it appears that you are experiencing fresh red blood in your stool and on toilet paper after bowel movements. This can be indicative of several conditions, some of which may require prompt medical evaluation.

Firstly, it's important to note that fresh red blood typically suggests that the bleeding is occurring in the lower gastrointestinal tract, which includes the rectum and anus. Common causes of rectal bleeding include hemorrhoids, anal fissures, diverticular disease, and colorectal polyps or cancer. Hemorrhoids and anal fissures are the most common causes, especially if the blood is bright red and associated with pain during bowel movements. These conditions can often be managed with conservative treatments, such as dietary changes, increased fluid intake, and topical medications.

However, given that you have experienced this bleeding more than once over the past few months, it is advisable to seek medical attention. You should consider visiting a gastroenterologist, a specialist in digestive system disorders. They can perform a thorough evaluation, which may include a physical examination, a digital rectal exam, and possibly further diagnostic tests such as a colonoscopy. A colonoscopy is particularly important as it allows for direct visualization of the colon and rectum, enabling the physician to identify any abnormalities, take biopsies if necessary, and treat certain conditions during the procedure.

In terms of when to seek immediate medical attention, you should do so if you experience any of the following symptoms:
1. Heavy Bleeding: If you notice a significant amount of blood (more than a few tablespoons) or if the bleeding persists.

2. Signs of Shock: Symptoms such as dizziness, fainting, or rapid heartbeat could indicate significant blood loss.

3. Severe Pain: If you experience severe abdominal pain or rectal pain that is not relieved by over-the-counter medications.

4. Changes in Bowel Habits: If you notice changes in your bowel habits, such as persistent diarrhea or constipation, or if you have unexplained weight loss.

5. Family History: If you have a family history of colorectal cancer or other gastrointestinal diseases, it is prudent to seek evaluation sooner rather than later.

In conclusion, while occasional rectal bleeding can be benign, especially if related to hemorrhoids or minor fissures, the recurrence of your symptoms warrants a medical evaluation to rule out more serious conditions. Make an appointment with a gastroenterologist to discuss your symptoms in detail and to undergo any necessary examinations. Early detection and treatment are key to managing any potential underlying issues effectively.
