After undergoing varicocele surgery and circumcision, the volume of ejaculation has significantly decreased?
Hello Doctor: I underwent surgery for varicocele and circumcision at the beginning of February.
Prior to the surgery, I often felt like I couldn't fully empty my bladder when urinating.
After the surgery, this sensation seems to have become more pronounced, as I frequently experience some dripping after urination, especially after a bowel movement or when sitting down.
Additionally, three weeks post-surgery, during my first ejaculation, the initial part was very watery, while the latter part resembled normal semen, being thicker but still in small volume.
Up until this week, during several attempts at ejaculation, I have felt the sensation of ejaculation, but the volume has been very minimal, almost watery, with only about 2 to 3 drops.
I would like to ask what this might indicate.
I have a follow-up appointment in two weeks, but I am still somewhat concerned, so I wanted to ask this question in advance.
Thank you for your help, Doctor!

Hello: It should not be related to the surgery; just ask the doctor during your follow-up appointment.
Wishing you good health.

Post-surgery concerns regarding reduced semen volume after varicocele repair and circumcision are quite common, and it is understandable that you are feeling anxious about these changes. Let’s break down your concerns and provide some insights based on medical knowledge.
Firstly, it is important to recognize that both varicocele repair and circumcision are surgical procedures that can have temporary effects on urinary and sexual function. Varicocele repair, in particular, is aimed at improving blood flow in the testicular veins, which can enhance sperm production and overall fertility in the long term. However, immediately after surgery, it is not uncommon for patients to experience changes in semen quality and urinary symptoms.
1. Reduced Semen Volume: After surgery, it is possible to experience a temporary reduction in semen volume. This can be attributed to several factors, including inflammation, changes in the prostate and seminal vesicles, or even psychological factors such as anxiety about the surgery and its outcomes. The fact that you noticed a difference in the consistency of your semen, with the first part being more watery and the latter part being thicker, is not unusual. Semen is composed of fluids from the seminal vesicles, prostate, and testicles, and changes in any of these glands can affect the overall volume and consistency.
2. Urinary Symptoms: The feeling of incomplete urination and the sensation of dripping after urination can also be related to the surgical procedures. Post-operative swelling or irritation in the pelvic area can lead to temporary changes in urinary function. It is essential to monitor these symptoms, as they may resolve over time. However, if they persist or worsen, it would be advisable to consult your healthcare provider for further evaluation.
3. Timing of Recovery: It is important to give your body time to heal after surgery. Surgical trauma can lead to inflammation and changes in the normal functioning of the reproductive and urinary systems. Typically, it may take several weeks to months for your body to fully recover and for semen volume and urinary function to return to baseline levels.
4. Follow-Up Care: Since you have a follow-up appointment scheduled in two weeks, it would be beneficial to discuss your concerns with your surgeon or urologist at that time. They may perform a physical examination and possibly recommend tests to assess your semen quality and urinary function.
5. Psychological Factors: Anxiety and stress about your surgical outcomes can also impact sexual function and semen production. It may be helpful to engage in relaxation techniques or speak with a mental health professional if you find that anxiety is affecting your recovery.
In conclusion, while it is concerning to experience reduced semen volume and urinary symptoms after surgery, these issues are often temporary and can improve with time. It is crucial to maintain open communication with your healthcare provider and report any persistent or worsening symptoms. They can provide personalized advice and reassurance based on your specific situation. Remember that recovery is a process, and it is important to be patient with your body as it heals.
