Managing Hypertension: Choosing Between Specialists in Northern and Central Taiwan - Cardiology

Referral related to hypertension?

In mid-February, I was diagnosed with malignant hypertension.
However, due to the Lunar New Year holiday, I sought medical attention in central Taiwan, where I underwent an electrocardiogram and cardiac catheterization, among other related tests.
I need to return to central Taiwan for a follow-up appointment, but I work in northern Taiwan.
I would like to know how to discuss with my doctor whether I should find another physician in the north to continue monitoring my condition or if I should primarily rely on the doctor in central Taiwan.
I am also concerned about the possibility of experiencing discomfort in the north in the future.
How should I address this? Thank you.

P, 30~39 year old female. Ask Date: 2018/03/07

Dr. He Dongjin reply Cardiology

Hello, please inform the physician directly that I would like to transfer my care back to the northern region due to my residence in the north.
I would appreciate your assistance in issuing a medical summary and whether you could recommend a physician at Ping An Changhua Hospital.
Thank you for your concern.

Managing hypertension, especially when it has been identified as malignant, requires careful consideration of your healthcare options. Given your situation, where you have received initial treatment in Central Taiwan but are now residing and working in Northern Taiwan, it is essential to approach this transition thoughtfully.

First and foremost, communication with your current healthcare provider in Central Taiwan is crucial. You should schedule a follow-up appointment to discuss your concerns about continuing care in Northern Taiwan. During this appointment, express your desire to find a local specialist who can monitor your condition more conveniently. Ask your doctor for a comprehensive summary of your medical history, including the results of your recent tests such as the electrocardiogram and catheterization. This information will be invaluable for any new physician you consult in the north.

When discussing your situation with your doctor, you might say something like, "I appreciate the care I've received here, but due to my work commitments in the north, I would like to explore options for continuing my treatment closer to home. Can you help me with a referral to a specialist in Northern Taiwan, and could you provide a summary of my medical records for them?"
In terms of finding a new specialist, consider looking for a cardiologist or a hypertension specialist who has experience managing malignant hypertension. You can check with your insurance provider for a list of covered specialists in your area. Additionally, you may want to seek recommendations from colleagues or friends who have had similar health issues.

It’s also important to establish a plan for managing your hypertension proactively. Since you have expressed concern about experiencing symptoms while in the north, ensure that you have a reliable method for monitoring your blood pressure at home. Regularly checking your blood pressure can help you identify any concerning trends early on. If you notice significant changes or if your blood pressure readings are consistently high, do not hesitate to seek immediate medical attention.

Furthermore, consider lifestyle modifications that can help manage your hypertension. These include maintaining a balanced diet low in sodium, engaging in regular physical activity, managing stress, and avoiding tobacco and excessive alcohol consumption. These changes can significantly impact your blood pressure and overall cardiovascular health.

In summary, transitioning your care from Central to Northern Taiwan involves clear communication with your current physician, obtaining your medical records, and finding a qualified specialist in your area. By taking these steps, you can ensure that your hypertension is managed effectively, minimizing the risk of complications. Remember, your health is a priority, and being proactive about your care will empower you to manage your condition successfully.
