Can pregnancy occur after abortion if ejaculation happens outside the body?

Hello Doctor, I took MISOPROSTOL for abortion on February 3, February 4, and February 5.
I had a follow-up appointment on February 15, and the doctor said that most of the tissue had cleared out, and the remaining tissue would be expelled with my next menstrual period.
After the Lunar New Year, I had sexual intercourse twice, both with withdrawal method, while the rest were with condoms.
The doctor mentioned that my period should arrive between March 3 and March 5, but it still hasn’t come.
However, I have experienced breast tenderness, and I usually feel itching in the vagina before my period.
Is it possible that I am pregnant? Or could it be that my period is delayed because last month was only 28 days?
Additionally, I have never had issues with constipation before; I used to have daily bowel movements.
Since the abortion, I have been experiencing severe constipation, regardless of how much water I drink or how much fruit I eat, with only minimal effect.
My stools are hard and dark in color.
Is this a side effect of the abortion? When can I expect to recover? Will my period come? Is it possible to become pregnant with the withdrawal method? Thank you, Doctor.

Hello, based on your description, there is a possibility of pregnancy, as the failure rate of using condoms correctly throughout intercourse is still about 5%.
Generally, it is not recommended to rely solely on condoms for contraception.
However, the absence of pregnancy cannot be determined by subjective symptoms.
It is advisable to take a urine test for pregnancy if your period is more than a week late.
The gastrointestinal symptoms you mentioned are likely not related to a miscarriage, and you may want to consult a gastroenterologist for evaluation and treatment.
Thank you! Wishing you safety and health.

After undergoing a medical abortion using Misoprostol, it is understandable to have concerns about the possibility of becoming pregnant again, especially after engaging in sexual activity shortly thereafter. The withdrawal method, also known as the "pull-out" method, is not a reliable form of contraception. While it may reduce the risk of pregnancy compared to unprotected intercourse, it does not eliminate it entirely. Pre-ejaculate fluid can contain sperm, and if any sperm enters the vagina, there is a chance of fertilization occurring.

Given that you mentioned having two instances of sexual intercourse with withdrawal and the rest with condom use, it is important to note that while condoms are generally effective in preventing pregnancy, they can fail due to breakage or improper use. Therefore, if you are experiencing symptoms such as breast tenderness and vaginal itching, it could indicate hormonal changes or an infection, but it also raises the possibility of pregnancy.

The absence of your expected menstrual period could be attributed to several factors. After an abortion, it may take some time for your menstrual cycle to regulate. Hormonal fluctuations can cause delays in menstruation, and stress or changes in your body can also play a role. If your previous cycles were consistently 28 days, a delay beyond a week or two could warrant a pregnancy test to rule out pregnancy.

Regarding your concerns about constipation following the abortion, it is not uncommon for individuals to experience gastrointestinal changes after such procedures. The hormonal changes, along with potential stress and dietary changes, can contribute to constipation. It is essential to maintain a balanced diet rich in fiber, stay hydrated, and engage in light physical activity if possible. If constipation persists, consider consulting a healthcare provider for further evaluation and management.

In summary, while the withdrawal method does carry a risk of pregnancy, the symptoms you are experiencing could be due to various factors, including hormonal changes or potential infection. It is advisable to take a home pregnancy test if your period does not arrive soon, and consult with your healthcare provider regarding your symptoms and any concerns about your menstrual cycle and gastrointestinal health. They can provide personalized guidance and support based on your specific situation.
