The visual field pulsates in sync with the heartbeat, exhibiting localized pulsations?
Hello doctor, I recently noticed that when I look up at the sky, the visual field in front of me pulsates in sync with my heartbeat (there's a flickering sensation).
This also happens when I squat down, but it stops after a few beats.
I have seen an ophthalmologist who said there is no issue, but since I have amblyopia in my left eye, this phenomenon is only observed in my right eye.
Which specialist should I consult next? Could there be something concerning in my brain? Thank you.

Seeing pulsating images that seem to synchronize with your heartbeat can be a perplexing and concerning experience. This phenomenon, often described as "pulsatile tinnitus" when it relates to sound, can also manifest visually, particularly in the context of vision. The symptoms you are experiencing may be attributed to several factors, including vascular dynamics, eye health, and neurological conditions.
Firstly, the visual pulsations you observe could be linked to the blood flow dynamics in your eyes. When you look up at the sky or change your position, the blood flow to your eyes may increase or fluctuate, causing the retinal blood vessels to pulsate. This can create a sensation of movement or flickering in your visual field, particularly if you have a condition like amblyopia (lazy eye), which you mentioned having in your left eye. The brain's interpretation of visual signals can be altered in individuals with amblyopia, leading to unusual visual experiences.
Additionally, the phenomenon could be related to the vitreous humor in your eye. As we age or due to certain conditions, the vitreous gel can become more liquid and may pull away from the retina, leading to visual disturbances such as flashes or floaters. If your right eye is compensating for the weakness in your left eye, it may be more sensitive to these changes, resulting in the pulsating effect you describe.
Another potential cause could be related to your cardiovascular health. If you have fluctuations in blood pressure or heart rate, these changes can affect how blood flows to the eyes, potentially leading to visual disturbances. It is essential to monitor your cardiovascular health, especially if you have a history of anxiety or hypertension, as these can exacerbate visual symptoms.
Regarding your concern about having something "bad" in your brain, it is understandable to feel anxious about unusual symptoms. However, visual disturbances that correlate with your heartbeat are not typically indicative of serious neurological issues. That said, if you are experiencing other symptoms such as headaches, dizziness, or changes in your overall health, it would be prudent to consult a neurologist or an ophthalmologist who specializes in neuro-ophthalmology. They can perform a comprehensive evaluation to rule out any underlying conditions.
In summary, while the pulsating images you see may be linked to normal physiological responses or eye health issues, it is crucial to seek further evaluation if you have ongoing concerns. Keeping a record of your symptoms, including when they occur and any associated factors (like changes in posture or stress levels), can be helpful for your healthcare provider. Regular eye exams and monitoring your cardiovascular health are also essential steps in managing your overall well-being. If your symptoms persist or worsen, do not hesitate to seek a second opinion or further testing to ensure your peace of mind and health.
