Anal or rectal issues

Hello Doctor: In the past few weeks, I have experienced a tearing sensation in my anus when using the restroom, although it hasn't been very severe.
Recently, I felt that the healing process seemed to be taking too long, so I started applying medication.
While applying the medication, I noticed a soft mass located at about the 12 o'clock position, which is directly in front of me, just beyond the ring of muscles around the anus.
The deeper I probe, the more it feels like something is obstructing my intestines.
There is no pain when I poke or press it, and it feels quite large, as if I can't reach the end of it.
I stopped probing after inserting two finger joints, but I don't feel a similar "mass" in other directions.
I have always had mild constipation issues.
I would like to ask if what I felt is a normal anatomical structure or if it could actually be hemorrhoids or another anal or rectal condition.
Thank you.

Based on your description, what you are feeling is more likely to be hemorrhoidal tissue.
It is recommended that you visit a surgical outpatient clinic for an examination to confirm.

When it comes to rectal discomfort, particularly when you experience a tearing sensation during bowel movements, it’s understandable to be concerned about the underlying cause. Based on your description, there are several possibilities to consider, including hemorrhoids, anal fissures, or other rectal conditions.

Firstly, hemorrhoids are swollen veins in the lower rectum or anus that can cause discomfort, itching, and sometimes bleeding. They can be classified as either internal or external. Internal hemorrhoids are located inside the rectum and usually do not cause pain, while external hemorrhoids can be felt around the anus and may cause significant discomfort, especially during bowel movements. The presence of a soft mass that you can feel may suggest the presence of external hemorrhoids, particularly if it is located near the anal opening.

On the other hand, anal fissures are small tears in the lining of the anus, which can cause sharp pain during and after bowel movements. They are often associated with constipation and passing hard stools. The tearing sensation you mentioned could be indicative of an anal fissure, especially if you have a history of constipation.

The fact that you described a soft mass located at the 12 o'clock position inside the rectum raises the possibility of several conditions. It could be a prolapsed hemorrhoid, which occurs when internal hemorrhoids protrude through the anal opening. Alternatively, it could be a rectal polyp or another type of growth, which would require further evaluation.

Given your history of mild constipation, it’s important to address this issue as it can exacerbate symptoms related to hemorrhoids and fissures. Increasing dietary fiber, staying hydrated, and possibly using stool softeners can help alleviate constipation and reduce strain during bowel movements.

It is crucial to consult a healthcare professional for a thorough examination. A physical examination, possibly including a digital rectal exam or an anoscopy, can provide more information about the mass you are feeling and help determine whether it is hemorrhoids, a fissure, or something else entirely. If necessary, your doctor may recommend imaging studies or a colonoscopy to rule out other conditions, especially if there are concerning symptoms such as significant pain, bleeding, or changes in bowel habits.

In summary, while your symptoms could be indicative of hemorrhoids or an anal fissure, it is essential to seek medical advice for an accurate diagnosis and appropriate treatment. Early intervention can help manage symptoms effectively and prevent complications. Please do not hesitate to reach out to a healthcare provider to discuss your symptoms and concerns in detail.
