Abdominal discomfort and bowel issues?

Hello, doctor.
I've been experiencing some discomfort in my abdomen over the past few days.
Sometimes I feel nauseous and have the urge to vomit.
I occasionally feel a warm sensation above my belly button, and there is a slight cramping pain on the right side.
My bowel movements have been softer or somewhat loose.
This afternoon, after using the restroom, I noticed a small streak of orange-red on the toilet paper (just a little bit, and I'm not sure if it's related to the carrot soup I had for lunch 2-3 hours ago).
I had a bilateral inferior turbinate radiofrequency ablation surgery a week ago, and the doctor prescribed erythromycin capsules along with other medications for pain relief, allergies, and expectorants.
The medication information mentioned that abdominal discomfort could be a possible side effect.
I'm wondering if this is related to the surgery or the medication, and if not, should I consider undergoing examinations such as an endoscopy or colonoscopy?

Dr. Ke Fangxu reply Surgery

Erythromycin commonly has gastrointestinal side effects.
Based on your description, it seems related to the medication; however, I do not recommend abruptly stopping the medication.
You should discuss with an otolaryngologist whether it is appropriate to discontinue the medication or switch to a different antibiotic.
If symptoms persist without improvement, please consult your physician for a prescription or consider undergoing a colonoscopy.

After undergoing surgery, particularly procedures involving the abdomen, it is not uncommon for patients to experience various forms of discomfort, including nausea, abdominal pain, and changes in bowel habits. In your case, the symptoms you describe—abdominal discomfort, nausea, localized warmth, mild pain on the right side, and changes in stool consistency—could be attributed to several factors related to your recent surgery.

Firstly, the discomfort you are feeling may be a result of the surgical procedure itself. Surgical interventions can lead to inflammation and irritation of the abdominal cavity, which may manifest as pain or discomfort in the days and weeks following the operation. The presence of nausea and changes in bowel habits, such as softer or looser stools, can also be linked to the medications you are taking post-surgery. Antibiotics, like the erythromycin you mentioned, can disrupt the normal balance of gut bacteria, potentially leading to gastrointestinal symptoms such as diarrhea or abdominal discomfort.

The orange-red coloration you observed on the toilet paper after wiping could be concerning, especially if it is not typical for you. While it is possible that this could be related to the consumption of carrot soup, as certain foods can temporarily alter the color of stool, it is essential to consider other possibilities as well. If the discoloration persists or is accompanied by other alarming symptoms such as significant abdominal pain, fever, or changes in bowel habits, it would be prudent to seek medical attention.

In terms of your surgical recovery, it is crucial to maintain open communication with your healthcare provider. If your symptoms are severe or worsening, or if you have any concerns about your recovery, it is advisable to consult your surgeon or primary care physician. They may recommend further evaluations, such as an endoscopy or colonoscopy, to rule out any complications or underlying issues that may require intervention.

In summary, while some degree of discomfort and changes in bowel habits can be expected after abdominal surgery, it is essential to monitor your symptoms closely. If you experience persistent or worsening symptoms, or if you have any concerns about the nature of your bowel movements, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. Regular follow-ups and open communication with your medical team are vital to ensure a smooth recovery and to address any potential complications early on.
