The sensation of the heart being squeezed?
Hello, doctor.
Since my teenage years, I have occasionally experienced palpitations, about once or twice a month.
Many years ago, I wore a monitor to check my heart rate and had an electrocardiogram, both of which showed no abnormalities.
At that time, the doctor diagnosed me with possible mitral valve prolapse.
I often feel fatigued and have low stamina.
Today, while working, I suddenly felt a bit dizzy but was still able to continue working.
Later, I began to feel a tightness in my chest, as if something was pressing down on it, and I had a sensation of my heart being squeezed.
I also experienced nausea, and I felt very uncomfortable but couldn't quite describe it.
The sensation of my heart being squeezed is very noticeable (this is the second time this month).
I would like to ask the doctor if there is something wrong with my heart.
Thank you.

Heart palpitations can be a concerning symptom, especially when accompanied by feelings of chest tightness, dizziness, and nausea. Your description of feeling as if your heart is being "gripped" and the associated symptoms raises important questions about the underlying causes and potential health implications.
Firstly, it is essential to understand that heart palpitations can arise from various factors, including physiological, psychological, and pathological causes. In your case, the history of mitral valve prolapse (MVP) is significant. MVP is a condition where the mitral valve does not close properly, which can lead to palpitations, especially during periods of stress or anxiety. It is not uncommon for individuals with MVP to experience symptoms such as palpitations, chest discomfort, and even dizziness.
Your symptoms of fatigue and physical weakness may also contribute to the sensation of palpitations. Fatigue can exacerbate feelings of anxiety, which in turn can lead to increased awareness of heartbeats or palpitations. Anxiety and panic attacks can manifest with physical symptoms, including dizziness, chest tightness, and a sensation of impending doom, which may explain your experience of feeling "gripped" in the chest.
The occurrence of dizziness during work, followed by chest tightness and nausea, could indicate a few possibilities. One potential cause is orthostatic hypotension, where blood pressure drops significantly upon standing or during physical exertion, leading to dizziness and palpitations. Another possibility is that these symptoms are related to anxiety or panic attacks, especially if they occur in stressful situations.
Given that you have had previous evaluations, including an electrocardiogram (ECG) and monitoring, which did not reveal any abnormalities, it is reassuring. However, if your symptoms persist or worsen, it would be prudent to seek further evaluation from a cardiologist. They may recommend additional tests, such as an echocardiogram, Holter monitor, or stress testing, to assess your heart's function and rule out any underlying cardiac conditions.
In the meantime, consider lifestyle modifications that may help alleviate your symptoms. Regular exercise, a balanced diet, adequate hydration, and stress management techniques, such as mindfulness or cognitive behavioral therapy, can be beneficial. If anxiety is a significant factor, discussing this with a mental health professional may provide you with coping strategies and treatment options.
Lastly, it is crucial to listen to your body. If you experience severe symptoms, such as persistent chest pain, shortness of breath, or fainting, seek immediate medical attention. Your health and well-being are paramount, and addressing these symptoms with a healthcare provider will help ensure that any potential issues are identified and managed appropriately.
