Sudden discomfort in the testicles extending to the lower abdomen?
Hello Dr.
Sun, my question is as follows: Two weeks ago, I went for a long bike ride and encountered a lot of poor-quality road surfaces.
At that moment, I felt a slight discomfort in my lower abdomen and groin area, but I thought it was just due to the rough ride, so I didn't pay much attention to it.
However, after returning home, I noticed that the mild pain from my lower abdomen to my groin did not improve; instead, it became intermittent.
Even walking requires me to adjust my posture slightly to feel more comfortable.
Later, when I stopped at a red light and put my left foot down, I felt a dull pain.
What could these symptoms indicate? Could it be varicocele?

Based on your description, this seems to be an acute occurrence, which is less likely to be varicocele.
If you are still concerned, you are welcome to visit our hospital, where we can arrange an ultrasound examination for you.
This will clearly show whether there is a varicocele or any injury to the testicles.

Understanding sudden discomfort in the testicles and lower abdomen can be concerning, especially when it follows a specific activity like cycling over rough terrain. Based on your description, it seems that you experienced initial discomfort that has since evolved into intermittent pain, particularly when walking or after putting weight on your left leg.
There are several potential causes for the symptoms you are experiencing, and while I cannot provide a definitive diagnosis, I can outline some possibilities that you may want to discuss with a healthcare professional.
1. Inguinal Hernia: One common cause of discomfort in the lower abdomen and testicular area is an inguinal hernia. This occurs when a portion of the intestine protrudes through a weak spot in the abdominal muscles, often leading to pain or discomfort, especially when engaging in physical activities or lifting heavy objects.
2. Varicocele: You mentioned the possibility of varicocele, which is an enlargement of the veins within the scrotum. This condition can cause a dull ache or discomfort in the testicles and is often more noticeable after prolonged standing or physical activity. Varicoceles are relatively common and can sometimes affect fertility, so it’s worth discussing with your doctor.
3. Testicular Torsion: Although less likely given the gradual onset of your symptoms, testicular torsion is a medical emergency that involves the twisting of the spermatic cord, cutting off blood supply to the testicle. This condition typically presents with sudden, severe pain and requires immediate medical attention.
4. Epididymitis: This is an inflammation of the epididymis, often caused by infection. Symptoms can include pain and swelling in the testicle, and it may be associated with urinary symptoms. If you have any fever or discharge, this could be a possibility.
5. Muscle Strain: Given that your discomfort began after cycling over rough terrain, it’s possible that you may have strained a muscle in the lower abdomen or groin area. This could lead to localized pain that might radiate to the testicular region.
6. Nerve Irritation: Sometimes, nerve irritation or entrapment in the lower back or pelvis can cause referred pain to the testicles or lower abdomen. This could be due to posture or the physical demands of cycling.
Given the persistence and nature of your symptoms, it is important to seek medical evaluation. A healthcare provider may perform a physical examination and possibly order imaging studies, such as an ultrasound, to assess for any structural abnormalities or other underlying conditions.
In terms of the urgency of your situation, if you experience any severe pain, swelling, or changes in urinary habits, you should seek immediate medical attention.
In summary, while there are several potential causes for your discomfort, a thorough evaluation by a healthcare professional is essential to determine the underlying issue and appropriate treatment. It’s important not to ignore these symptoms, especially since they have persisted and are affecting your daily activities.
