Glans issues
Hello doctor, I had a circumcision, and last year around mid-year, I noticed peeling on the left side of the glans.
Over time, the area gradually expanded.
I researched online and found symptoms similar to balanitis, so I bought an ointment to apply.
Eventually, it improved, but approximately every two to three weeks, or once a month, the same symptoms reappear in a similar location.
This has been recurring for six months.
Whenever I experience symptoms, I start applying the ointment, but it usually takes two to three weeks for the symptoms to improve.
The symptoms typically begin with a small red patch, the skin becomes rough, and then it starts to peel.
It expands gradually to an area slightly smaller than a coin, followed by more peeling, roughness, and slight itching.
When the ointment finally heals it, the skin returns to being as smooth as the unaffected areas of the glans, but after a few weeks, the symptoms reappear.
I visited a urologist at a major hospital once, and the doctor diagnosed it as balanitis.
I showed him the ointment I was using, and he said it was fine, then prescribed me another ointment that was similar.
Recently, it has recurred again, and within six months, it has flared up five or six times.
I have not engaged in sexual intercourse.
I have a few questions for the doctor: 1.
What methods can completely eradicate this condition? 2.
During the period of symptom flare-up, is it advisable to refrain from masturbation? 3.
Even if the skin appears to have healed, should I continue applying the ointment for a while to ensure complete resolution? 4.
When peeling occurs, should I avoid picking at it?

Hello: 1) Due to recurring issues, you may need to see a urologist for an examination.
Be sure to inform the physician about the recurrence (also inquire if there is a history of diabetes?).
2) For now, avoid it.
3) Theoretically, the skin condition has improved—just keep it clean and dry.
4) No, just clean with plain water and then dry it off.
It will resolve on its own.
Wishing you good health.

More Info
Recurrent glans issues, particularly those resembling balanitis (inflammation of the glans penis), can be frustrating and concerning. Based on your description, it seems you have been experiencing recurring symptoms characterized by peeling skin, redness, rough texture, and mild itching. Here’s a comprehensive overview of potential causes, treatment options, and answers to your specific questions.
Causes of Recurrent Glans Issues
1. Balanitis: This is the most common cause of inflammation of the glans. It can be due to infections (bacterial, fungal, or yeast), irritants (such as soaps, lotions, or condoms), or skin conditions (like eczema or psoriasis).
2. Poor Hygiene: Inadequate cleaning can lead to the accumulation of smegma, which can irritate the glans and lead to inflammation.
3. Allergic Reactions: Sometimes, products used in the genital area can cause allergic reactions, leading to symptoms similar to what you described.
4. Underlying Conditions: Conditions such as diabetes can predispose individuals to infections and skin issues, including recurrent balanitis.
Treatment Options
1. Topical Treatments: Since you have been using topical ointments, it’s essential to ensure that they are appropriate for your condition. Over-the-counter antifungal creams or hydrocortisone may help, but it’s crucial to follow a healthcare provider's recommendations.
2. Proper Hygiene: Maintaining good hygiene is vital. Gently clean the area with mild soap and water, and ensure it is thoroughly dried afterward.
3. Avoid Irritants: Identify and avoid any products that may be causing irritation. This includes certain soaps, lotions, or even laundry detergents.
4. Consult a Specialist: If the problem persists, consider seeing a dermatologist or a urologist specializing in skin conditions. They may perform a skin biopsy or other tests to rule out other underlying issues.
Answers to Your Specific Questions
1. Can it be completely eradicated?: The potential for complete resolution depends on the underlying cause. If it’s due to an irritant or a treatable infection, it may be possible to resolve it completely. However, if it’s related to a chronic condition or skin disorder, management may be necessary rather than a complete cure.
2. Can you masturbate during flare-ups?: It’s generally advisable to avoid any activities that may irritate the area during a flare-up, including masturbation. This can prevent further irritation and allow the area to heal more effectively.
3. Should you continue treatment even after symptoms improve?: Yes, it’s often recommended to continue using the prescribed treatment for a short period after symptoms have resolved to ensure that the underlying issue is fully addressed. However, consult your healthcare provider for specific guidance tailored to your situation.
4. Should you avoid peeling skin?: It’s best not to forcibly remove peeling skin, as this can lead to further irritation or infection. Allow the skin to heal naturally. If the peeling is excessive or accompanied by pain, consult a healthcare provider.
Conclusion
Recurrent glans issues can be challenging, but with proper care and management, you can minimize symptoms and improve your quality of life. It’s essential to work closely with your healthcare provider to identify the underlying cause and develop an effective treatment plan. If symptoms persist or worsen, further evaluation may be necessary to rule out other conditions. Remember, maintaining good hygiene and avoiding irritants are key components in managing this issue effectively.
