Testicular Discomfort After Bicycle Injury: Possible Causes and Solutions - Urology

Testicular issues?

A few weeks ago, I hit my left testicle while riding a bicycle.
Since then, I have been experiencing discomfort whether sitting or lying down, and I feel a bit awkward while walking.
It seems like my testicles are positioned closer together in the middle of my legs, with one testicle noticeably in front of the other.
The left testicle also tends to retract on its own, and sometimes I feel a tingling or aching sensation in my left thigh.
After ejaculation, the discomfort in my testicle increases, accompanied by some pain.
Could you please tell me what might be the cause? Thank you.

Mr. Lin, 10~19 year old female. Ask Date: 2018/04/14

It has been several weeks, so you should seek medical attention.
Wishing you good health.

Experiencing discomfort in the testicular area following a bicycle injury can be concerning and warrants careful evaluation. Based on your description, there are several potential causes for your symptoms, and understanding these can help guide you toward appropriate management.

1. Trauma to the Testicles: The immediate cause of your discomfort is likely related to the trauma sustained during the bicycle accident. Direct impact to the testicles can lead to bruising, swelling, or even more severe injuries such as testicular torsion or rupture. The sensation of the left testicle being higher or closer to the midline could indicate swelling or a hematoma (a localized collection of blood outside of blood vessels) that may have altered the normal positioning of the testicle.

2. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. Symptoms typically include sudden, severe pain, swelling, and sometimes nausea. While you mentioned discomfort rather than severe pain, it’s important to rule this out, especially given the changes in position and discomfort during ejaculation.

3. Epididymitis: This is an inflammation of the epididymis, which can occur following trauma or infection. Symptoms may include swelling, tenderness, and discomfort, particularly during activities like sitting or sexual activity. The accompanying sensation of "tingling" or "numbness" in the thigh could suggest nerve involvement or referred pain from the testicular area.

4. Nerve Irritation: The discomfort in your left thigh could be due to irritation of the nerves that innervate the groin and thigh area. Trauma can lead to inflammation that may affect nearby nerves, causing sensations of tingling or numbness.

5. Referred Pain: Sometimes, pain in the testicular area can be referred from other structures, such as the lower back or pelvis. If there was any impact to your pelvis or lower back during the accident, this could also contribute to your symptoms.

Recommended Actions:
- Seek Medical Evaluation: It is crucial to consult a healthcare provider, preferably a urologist, who can perform a thorough examination. They may conduct an ultrasound to assess blood flow to the testicles and check for any abnormalities such as a hematoma or torsion.

- Rest and Avoid Aggravating Activities: Until you can be evaluated, it’s advisable to avoid activities that exacerbate your discomfort, such as cycling or heavy lifting.

- Pain Management: Over-the-counter pain relievers like ibuprofen or acetaminophen may help alleviate discomfort, but consult your doctor before taking any medication, especially if you have other health conditions or are taking other medications.

- Monitor Symptoms: Keep track of any changes in your symptoms, such as increased pain, swelling, or changes in urination or sexual function. If you experience severe pain, nausea, or vomiting, seek emergency medical attention immediately.

In summary, while there are several potential causes for your testicular discomfort following your bicycle injury, a thorough evaluation by a medical professional is essential to determine the exact cause and appropriate treatment. Early intervention can help prevent complications and ensure a better recovery.
