Abdominal issues?
Hello Doctor, this issue has been affecting me for about a month.
I have been feeling a tight sensation in my left abdomen, roughly two finger widths below my last rib.
I can't quite describe it, but it feels like pressure.
When I inflate my stomach, that area seems slightly more protruded than the right side, but it doesn't hurt.
When I press down with my hand, it feels similar to muscle tissue! When I lean back, that area feels tight.
I've also noticed an increase in my bowel movements; sometimes I have diarrhea, while other times it's normal.
My appetite is normal, and I haven't lost any weight.
I find myself burping and passing gas more than before.
I'm not sure if this has affected me mentally; I'm someone who easily gets nervous and anxious.
If I don't think about it, I feel a bit better, but it still feels strange.
When I do think about it, I become quite uncomfortable.
Since I'm abroad and it's inconvenient to seek medical care, I've been discussing with my mom whether I should return to Taiwan to see a doctor.
I'm really worried that something might be wrong.
Do you have any advice, Doctor? Thank you.

More Info
Abdominal discomfort can be a complex issue, often influenced by a variety of factors, including physical health, psychological well-being, and lifestyle habits. Based on your description, it seems you are experiencing a sensation of tightness or pressure in the left upper quadrant of your abdomen, along with changes in bowel habits and increased gas. While these symptoms can be concerning, it’s important to approach them systematically to identify potential causes and appropriate solutions.
Common Causes of Abdominal Discomfort
1. Gastrointestinal Issues: The symptoms you describe, such as changes in bowel habits (increased frequency and occasional diarrhea), could suggest a gastrointestinal condition. Common issues include:
- Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can cause symptoms like abdominal pain, bloating, and changes in bowel habits. Stress and anxiety often exacerbate IBS symptoms.
- Gastritis or Acid Reflux: Inflammation of the stomach lining or acid reflux can lead to discomfort and a feeling of pressure in the abdomen.
- Food Intolerances: Lactose intolerance or gluten sensitivity can lead to bloating, gas, and changes in bowel habits.
2. Musculoskeletal Factors: The sensation of tightness could also be related to muscle tension or strain in the abdominal wall. Stress and anxiety can lead to muscle tension, which may manifest as discomfort in the abdomen.
3. Psychological Factors: Given your mention of anxiety, it’s worth noting that psychological factors can significantly influence physical symptoms. Anxiety can lead to heightened awareness of bodily sensations, which may exacerbate feelings of discomfort. This is sometimes referred to as somatic symptom disorder, where psychological distress manifests as physical symptoms.
4. Other Considerations: While less common, conditions such as hernias, kidney stones, or even splenic issues could cause localized discomfort. However, these are typically accompanied by more severe symptoms.
Recommendations for Management
1. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated factors (like food intake or stress levels). This can help identify patterns and triggers.
2. Dietary Adjustments: Consider evaluating your diet for potential irritants. A diet rich in fiber, low in processed foods, and mindful of potential allergens (like dairy or gluten) may help alleviate symptoms. Staying hydrated is also crucial.
3. Stress Management: Since you mentioned being prone to anxiety, incorporating stress-reduction techniques such as mindfulness, meditation, or yoga may help alleviate both psychological and physical symptoms. Regular physical activity can also be beneficial.
4. Consult a Healthcare Provider: If symptoms persist or worsen, it’s advisable to seek medical attention. While you mentioned difficulties accessing healthcare abroad, consider telemedicine options or local clinics that may provide the necessary evaluations. A healthcare provider may recommend tests such as blood work, imaging studies, or gastrointestinal evaluations to rule out any serious conditions.
5. Consider Returning Home for Care: If your symptoms are significantly impacting your quality of life and you feel more comfortable seeking care in Taiwan, it may be worth planning a visit. Having access to familiar healthcare providers can ease anxiety and ensure you receive appropriate evaluations and treatments.
Conclusion
Abdominal discomfort can stem from various causes, and understanding the interplay between physical and psychological factors is crucial. While your symptoms may be benign, it’s essential to monitor them closely and seek professional advice if they persist. Remember, taking proactive steps towards managing both your physical and mental health can lead to significant improvements in your overall well-being.
