Erectile Dysfunction: Overcoming Challenges After Frequent Stimulation - Urology

The penis becomes flaccid shortly after manual stimulation?

Hello doctor, I am 36 years old and have been frequently watching adult films for a long time.
However, when I masturbate, if I stop the continuous friction, my erection goes soft after about 10 seconds.
I have tried exercising for a few months, but there has been no improvement.
I am aware of the seriousness of this issue.
Last year, I visited a urologist who conducted tests and found that I do not have hypertension, diabetes, or any other related issues; all my indicators were normal.
However, I was eager to improve, so I underwent shockwave therapy, but there was no significant progress.
Eventually, the doctor prescribed Viagra, and I suddenly realized that I could achieve an erection without friction.
I have not stimulated my brain in this way for quite some time.

In the past few months, while watching adult films, I no longer engage in habitual viewing; instead of automatically reaching for friction, I try to focus on feeling aroused and achieving an erection naturally.
I have noticed that if I allow my body to achieve an erection on its own, it can last between 2 to 3 minutes.
However, I can no longer achieve an erection through friction as frequently as before; it may take two days or even several days before I can achieve an erection on my own.
Currently, I only masturbate after my penis has achieved an erection naturally.
Sometimes, when I want to masturbate after watching a film, I find that I cannot achieve an erection.
It may take a day or a few days for my body to be able to achieve an erection again.
Additionally, if I simply use my hand for friction, it still goes soft after 10 seconds.
I want to change these two situations; what should I do?

Hello: Your erectile blood vessels should not have any issues; rather, it seems that long-term behaviors have led to a habitual response to sexual stimulation.
Just like some long-standing lifestyle habits are difficult to change, I recommend that you be patient, reduce the frequency of watching adult films, and ensure you engage in regular exercise and maintain a light diet.
This may help you overcome the reliance on manual stimulation to achieve an erection.
Wishing you good health.

Erectile dysfunction (ED) is a complex issue that can arise from a variety of factors, including psychological, physiological, and lifestyle influences. In your case, it seems that frequent stimulation through pornography and masturbation has led to a form of desensitization, where your body has become accustomed to a specific type of stimulation, making it difficult to achieve and maintain an erection without that stimulation.

Understanding the Issue
1. Psychological Factors: The brain plays a crucial role in sexual arousal. When you frequently watch pornography, your brain may develop a preference for the visual and immediate stimulation it provides. This can lead to difficulties in achieving arousal in more natural settings, such as with a partner or even through self-stimulation without the same level of external stimulation.

2. Physiological Factors: While you mentioned that your medical tests showed no significant issues, it's important to recognize that erectile function is influenced by blood flow, nerve function, and hormonal levels. The fact that you experienced improvement with Viagra (sildenafil) suggests that your body can respond to sexual stimulation but may require more direct or intense stimulation to do so.

3. Desensitization: Over time, your body may become desensitized to the type of stimulation you are used to, leading to difficulties in achieving an erection without the same level of stimulation. This can create a cycle where you feel the need to engage in more frequent or intense stimulation to achieve the same results.

Steps to Overcome the Challenges
1. Limit Pornography Use: Reducing or eliminating pornography consumption can help reset your brain's response to sexual stimuli. This may take time, but it can help you regain sensitivity to more natural forms of arousal.

2. Practice Mindfulness: Engaging in mindfulness techniques can help you become more aware of your body and its responses. This can include focusing on physical sensations during intimate moments without the pressure of achieving an erection.

3. Gradual Exposure: Instead of relying on immediate stimulation, try to gradually expose yourself to sexual situations without the use of pornography. This could involve spending time with a partner in intimate settings, focusing on emotional connection rather than just physical stimulation.

4. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve erectile function. These exercises help enhance blood flow and improve control over erections.

5. Consult a Specialist: Since you have already seen a urologist, consider discussing your concerns with a mental health professional who specializes in sexual health. They can provide therapy options that address the psychological aspects of ED.

6. Healthy Lifestyle Choices: Continue to engage in regular physical activity, maintain a balanced diet, and ensure you are managing stress effectively. These factors can all contribute to improved sexual health.

7. Medication Management: If you find that you still struggle with achieving erections, discuss with your doctor the possibility of adjusting your medication or exploring other options that may be available to you.

Conclusion
Erectile dysfunction can be a challenging issue, especially when it is influenced by habits and psychological factors. By taking proactive steps to address these issues, including reducing reliance on pornography, practicing mindfulness, and seeking professional help, you can work towards improving your erectile function and overall sexual health. Remember, it is essential to be patient with yourself as you navigate this process, and seeking support from healthcare professionals can provide valuable guidance along the way.
