Scattering issues
Hello doctor, my son is over four years old.
He was diagnosed with amblyopia last October.
He has been visiting the clinic for check-ups in October, November, December, and April, during which he receives atropine eye drops; sometimes he gets them on the same day, and other times he takes them home to use for four days before the next check-up.
I would like to know if using atropine almost every month will have any effects on a child's eyes.
Additionally, his left eye was initially able to see 0.8, but during the last check-up, it was only 0.6.
Is this due to prolonged patching making his lazy eye worse, or is it normal for vision to fluctuate? Thank you.

Hello: Does using eye drops every month have an impact on children's eyes? It is concerning that the left eye, which originally had a vision of 0.8, was later found to have only 0.6.
The information is insufficient to make a conclusion.
Wishing you peace and health.
Sincerely, Dr.
Tsai Wen-Yuan, Director of the Ophthalmology Department, Taoyuan Hospital, Ministry of Health and Welfare.

The use of cycloplegic eye drops, such as tropicamide, in children, especially those diagnosed with amblyopia (lazy eye), is a common practice in pediatric ophthalmology. These drops temporarily paralyze the ciliary muscle of the eye, which helps in measuring refractive errors accurately by preventing the eye from accommodating (focusing). However, the frequent use of these drops raises concerns among parents regarding their potential impact on a child's vision development.
Firstly, it is important to understand that cycloplegic agents are generally safe when used as prescribed by an eye care professional. They are often employed to ensure accurate refraction measurements, which is crucial for determining the appropriate corrective lenses for children with refractive errors. In your case, the drops are likely being used to monitor and manage your child's amblyopia effectively.
Regarding your concern about the frequency of administration, using cycloplegic drops almost every month is not uncommon in a clinical setting, especially if the child is undergoing treatment for amblyopia. The drops can help in assessing the true refractive error without the influence of accommodation, which can fluctuate in children. However, it is essential to follow the guidance of your child's ophthalmologist, who will consider the specific needs and responses of your child to the treatment.
As for the observed decrease in visual acuity from 0.8 to 0.6 in the left eye, several factors could contribute to this change. It is possible that the eye may appear "lazy" due to prolonged occlusion therapy, which is often used to treat amblyopia by forcing the weaker eye to work harder. This can sometimes lead to temporary fluctuations in vision, as the brain and eyes adjust to the changes in visual input. Additionally, children's vision can naturally fluctuate due to various reasons, including fatigue, attention, and even emotional state during the examination.
It is also worth noting that amblyopia treatment requires consistent effort and monitoring. If the left eye is not being used adequately, it may not develop as well as it could, leading to a decrease in visual acuity. This phenomenon is sometimes referred to as "visual neglect," where the brain favors the stronger eye, causing the weaker eye to become less effective over time.
In conclusion, while the frequent use of cycloplegic drops is generally safe and beneficial for managing amblyopia, it is crucial to maintain open communication with your child's ophthalmologist. They can provide tailored advice based on your child's specific situation and ensure that the treatment plan is effective. Regular follow-ups and adherence to prescribed therapies, including occlusion therapy, are vital for improving visual outcomes in children with amblyopia. If you have any concerns about your child's vision or the treatment plan, do not hesitate to discuss them with your healthcare provider.
