Pain extending from the right posterior neck to the right orbital region?

Hello Dr.
Jiang, a few days ago I started experiencing a dull pain in the right posterior neck, which has continued for the past two days, typically starting around noon.
The pain is localized in the right posterior neck (Fengchi point) and feels dull.
Stretching provides some relief, but afterwards, the pain gradually intensifies, radiating from the temple to the right eye socket.
At its worst, the pain feels similar to that of sensitive teeth, but it is confined to the right side.
During the most painful moments, even my right masseter muscle hurts when I clench my teeth.
The pain lasts for about three hours, and when it starts, I take pain medication (Rode Medipill), but it doesn't seem to help, and I do not feel nauseous.
Two years ago, I experienced a similar headache with comparable symptoms.
I underwent examinations and took pain medication, but it was ineffective; however, I felt better after getting a new pair of glasses (I had changed my glasses about a month before the onset of that pain).
I would like to ask Dr.
Jiang: 1.
What could be the cause of my current headache symptoms? 2.
Could it be related to my glasses? However, I haven't changed my glasses and am still using the same pair.
Thank you, Dr.
Jiang, for taking the time to read and respond.

Mr. Wu, 20~29 year old female. Ask Date: 2018/04/23

Dr. Jiang Junyi reply Neurology

Hello:
1.
What could be the possible causes of the current headache symptoms?
A: 1.
You mentioned that the symptoms improve after stretching, which may indicate a myofascial issue.
2.
Additionally, consider cluster headaches, which are more common in males and typically occur in the spring and fall.
The headaches are usually short-lived and severe, presenting as unilateral pain located beneath the eye, with pain and tearing on the same side at the temple or cheek.
Clinically, oxygen inhalation therapy may be administered to alleviate symptoms within 10 minutes.
2.
Could it be related to my glasses? I haven't changed my glasses and have been using the same ones.

A: If your symptoms occur after prolonged eye use, it may be worth considering a refractive issue (such as myopia, hyperopia, or astigmatism).
I recommend consulting with a neurologist and an ophthalmologist for evaluation and treatment.
Wishing you good health and peace.
Dr.
Tao, Shen Nei, cares about you.

More Info

Based on your description of the symptoms, it seems that you are experiencing a combination of neck pain and headaches that radiate to the eye area, particularly on the right side. This type of pain can be quite complex and may arise from several potential causes.
1. Muscle Tension and Strain: One of the most common causes of pain in the neck that radiates to the head and eyes is muscle tension. This can occur due to poor posture, especially if you spend long hours looking at screens or using your phone. The muscles in your neck and shoulders can become tight and fatigued, leading to referred pain in the head and eyes. Stretching may provide temporary relief, but if the underlying tension persists, the pain can return.

2. Cervicogenic Headaches: These headaches originate from issues in the cervical spine (the neck) and can cause pain that radiates to the forehead, temples, and behind the eyes. The symptoms you describe, including the sensation of tightness and pain that can be exacerbated by certain movements or positions, align with cervicogenic headaches.
3. Nerve Irritation or Compression: If there is any irritation or compression of the nerves in the neck, it can lead to pain that radiates to the head and eyes. This could be due to herniated discs, bone spurs, or other spinal issues.
4. Eye Strain: Although you mentioned that you have not changed your glasses recently, if your prescription is not optimal or if you are experiencing eye strain from prolonged screen time, this could contribute to headaches and discomfort around the eyes.
5. Sinus Issues: Sometimes, sinus infections or sinusitis can cause pain that radiates to the forehead and around the eyes. If you have any nasal congestion or other sinus symptoms, this could be a factor.

6. Migraine or Tension-Type Headaches: Given your history of similar symptoms, it is possible that you are experiencing a migraine or tension-type headache. These types of headaches can be triggered by various factors, including stress, lack of sleep, or changes in routine.

Recommendations:
- Posture Correction: Pay attention to your posture, especially when using electronic devices. Ensure that your screen is at eye level and take regular breaks to stretch and move around.

- Neck Exercises: Gentle neck stretches and exercises can help alleviate tension. Consider consulting a physical therapist for tailored exercises.

- Eye Care: If you suspect your glasses may be contributing to your discomfort, it might be worth getting a comprehensive eye exam to ensure your prescription is still appropriate.

- Pain Management: Over-the-counter pain relievers can help, but if they are not effective, consult a healthcare provider for alternative options.

- Consult a Specialist: Given the recurring nature of your symptoms, it would be wise to consult a neurologist or an orthopedic specialist who can evaluate your neck and head pain more thoroughly.

- Stress Management: Incorporating relaxation techniques such as yoga, meditation, or deep-breathing exercises can help reduce overall tension and stress, which may alleviate some of your symptoms.

In summary, your symptoms could be due to a combination of muscle tension, potential nerve issues, or eye strain. It is essential to monitor your symptoms and seek professional medical advice if they persist or worsen, as a thorough evaluation may be necessary to determine the underlying cause and appropriate treatment.
