It feels like there is something in the rectum?

Hello Doctor, I just had hemorrhoid surgery, but on the third day, my stool was too hard, which seems to have compromised the surgery.
While applying the ointment, I inserted my finger a bit and felt something the size of a ping pong ball in the rectum.
I'm not sure what it is.
Which specialty should I consult? Should I go to urology or return to my original doctor for an examination?

Yang Gui, 20~29 year old female. Ask Date: 2018/04/25

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience unusual findings or sensations in the rectal area. In your case, discovering a lump the size of a ping pong ball in the rectum three days post-surgery raises several concerns that warrant further evaluation.

First and foremost, it is essential to return to the surgeon who performed the hemorrhoid surgery for a follow-up appointment. The surgeon will have the most comprehensive understanding of your surgical procedure and can assess whether the lump is a normal post-operative finding or something that requires further investigation. It is crucial to communicate all your symptoms, including the hardness of your stool and any discomfort you may be experiencing, as these details can significantly aid in diagnosis.

The lump you described could potentially be a few different things. One possibility is that it could be a hematoma, which is a localized collection of blood outside of blood vessels, often resulting from surgical trauma. Hematomas can sometimes feel firm and may take time to resolve. Another possibility is that it could be a prolapsed hemorrhoid, which can occur if the tissue that was surgically treated has not healed properly or if there was excessive strain during bowel movements. Additionally, it could be a sign of an abscess or infection, especially if there is associated pain, redness, or discharge.

Regarding your concern about the hardness of your stool, it is essential to manage this to prevent further complications. Hard stools can lead to straining during bowel movements, which can put pressure on the surgical site and potentially lead to complications such as re-bleeding or additional hemorrhoids. To alleviate this, consider increasing your fluid intake, consuming a high-fiber diet, and possibly using stool softeners or laxatives as recommended by your healthcare provider.

As for which specialist to consult, your best course of action is to return to the colorectal surgeon who performed your hemorrhoid surgery. They will be able to perform a thorough examination, possibly including a digital rectal exam or an anoscopy, to visualize the area and determine the nature of the lump. If necessary, they may refer you to a gastroenterologist or another specialist for further evaluation.

In summary, it is crucial to follow up with your surgeon regarding the unusual finding in your rectum after hemorrhoid surgery. They will provide the most accurate assessment and guide you on the next steps. In the meantime, focus on managing your bowel habits to prevent hard stools and straining, which can complicate your recovery. Always prioritize open communication with your healthcare providers about any new or concerning symptoms you experience.
