Semen issues

Hello Doctor, I would like to ask you about something.
Every time I masturbate and ejaculate, the semen feels not very viscous, almost like water, and I almost always have issues with clumping.
I searched online and found that clumping might be caused by not masturbating for a long time, but I have tried it the next day and still experienced clumping.
I would like to know what symptoms this might indicate and whether it could lead to infertility issues.
Additionally, when I was younger, I was kicked in the testicles; could that be a contributing factor? Those are my main questions.
Thank you, Doctor.

A-Kai, 20~29 year old female. Ask Date: 2018/04/25

It is recommended to visit a urology clinic for an examination to determine sperm quality.

Understanding semen consistency and its implications for fertility is a common concern among men. The characteristics of semen, including its viscosity and the presence of clumps or "chunks," can vary significantly from person to person and can be influenced by several factors, including hydration, frequency of ejaculation, and overall health.

Firstly, the consistency of semen can be affected by how often a man ejaculates. If there is a long gap between ejaculations, the semen may become thicker and more viscous due to the accumulation of sperm and seminal fluid. Conversely, frequent ejaculation can lead to a more watery consistency. However, if you are experiencing a consistently watery consistency with clumping, it may be worth investigating further.

The presence of clumps in semen can sometimes be attributed to the presence of spermatozoa that have not fully liquefied after ejaculation. Normally, semen should liquefy within 15 to 30 minutes after ejaculation, allowing sperm to swim freely. If clumping persists beyond this timeframe, it could indicate an issue with the seminal fluid or the prostate gland, which produces a significant portion of the seminal fluid. Conditions such as prostatitis (inflammation of the prostate) or seminal vesiculitis (inflammation of the seminal vesicles) could potentially lead to changes in semen consistency.

Regarding your concern about infertility, while semen consistency can play a role in fertility, it is not the sole determinant. A semen analysis, which evaluates sperm count, motility (the ability of sperm to move), morphology (shape), and overall volume, is essential to determine fertility potential. If the semen analysis shows abnormalities, it could indicate potential fertility issues. However, many men with less-than-ideal semen characteristics can still father children, so it is important not to jump to conclusions based solely on consistency.

As for the incident from your childhood where your testicles were kicked, trauma to the testicles can lead to various issues, including potential damage to the sperm-producing cells. However, unless there was a significant injury that resulted in surgical intervention or severe pain, it is unlikely that this incident alone would cause long-term fertility issues. If you have concerns about past trauma affecting your fertility, discussing this with a healthcare provider can provide clarity.

In summary, while the consistency of your semen may raise questions, it is essential to consider a comprehensive evaluation of your reproductive health. If you are concerned about your fertility, it is advisable to consult a urologist or a fertility specialist who can perform a semen analysis and provide personalized recommendations based on your specific situation. They can also assess any potential underlying conditions that may be affecting your semen quality and overall reproductive health. Remember, many factors contribute to fertility, and addressing them holistically is key to understanding your reproductive potential.
