Delayed menstruation?
Hello, doctor.
My last menstrual period was from March 29 to April 4.
According to the app, my estimated ovulation date was April 17.
I had sexual intercourse with my boyfriend on April 29, but we used a condom the entire time and he did not ejaculate inside me.
My next period was expected on May 1, but it is currently 8 days late.
I took a pregnancy test on the fourth day of the delay, May 5, and it showed no signs of pregnancy.
I would like to know if there is still a possibility of being pregnant or if there could be other issues causing this delay.
Thank you.
Niming, 20~29 year old female. Ask Date: 2018/05/09

Hello, the factors affecting the menstrual cycle are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraception, diet, polycystic ovary syndrome, pregnancy, breastfeeding, and more.
Even with consistent and correct condom use, the failure rate is still around 5%.
Therefore, it is generally not recommended to rely solely on condoms for contraception.
If your menstrual period does not arrive as expected, it is advisable to consult a gynecologist to check for hormonal imbalances or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

More Info
Understanding delayed menstruation can be a source of concern for many women, especially when there is a possibility of pregnancy. In your case, you have provided a timeline of your menstrual cycle, sexual activity, and the use of protection, which is helpful in assessing the situation.
First, let's break down the timeline you've shared. Your last menstrual period started on March 29 and ended on April 4. Based on a typical 28-day cycle, ovulation would likely occur around April 17. You mentioned having sexual intercourse on April 29, which is about 12 days after ovulation. Since you used a condom and there was no ejaculation inside, the risk of pregnancy is significantly reduced, but not entirely eliminated. Condoms can occasionally fail due to breakage or slippage, and pre-ejaculate fluid can also contain sperm, although the risk is lower.
Now, regarding your delayed menstruation, which is currently 8 days late, there are several potential causes to consider:
1. Pregnancy: Although you tested negative for pregnancy on May 5, it’s important to note that home pregnancy tests are most accurate when taken after a missed period. If you ovulated later than expected, or if implantation occurred later, it might not have been detectable yet. If your period does not start soon, consider retesting a few days later.
2. Stress: Emotional or physical stress can significantly impact your menstrual cycle. Stress can lead to hormonal imbalances that may delay ovulation or menstruation.
3. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can cause irregularities in your menstrual cycle. If you have a history of irregular periods, this could be a contributing factor.
4. Weight Changes: Significant weight loss or gain can also affect your menstrual cycle. If you have experienced any changes in your weight recently, this could be a factor.
5. Illness: Any recent illness or changes in your health can impact your cycle. Conditions such as infections or other medical issues can lead to delayed menstruation.
6. Medications: If you have started or stopped any medications recently, particularly hormonal contraceptives, this could affect your cycle.
Given that you have already taken a pregnancy test with a negative result, and considering the use of protection during intercourse, it is less likely that you are pregnant. However, if your period does not arrive soon, or if you experience any unusual symptoms (such as severe pain, heavy bleeding, or other concerning signs), it would be advisable to consult with a healthcare provider. They can perform further evaluations, including blood tests or ultrasounds, to determine the cause of your delayed menstruation.
In conclusion, while pregnancy is a possibility, there are many other factors that could be contributing to your delayed period. Monitoring your symptoms and maintaining open communication with your healthcare provider will be key in addressing your concerns and ensuring your reproductive health. If you have any further questions or if your situation changes, do not hesitate to seek medical advice.
