Phimosis
Hello, I am an 18-year-old high school student preparing to apply to the police academy.
I have heard that the academy has communal showers, and I suddenly became aware of my foreskin issue.
I am afraid of being laughed at when I go there.
Currently, I cannot retract my foreskin regardless of whether I have an erection.
When I am erect, I can push the foreskin back to see the glans, but I cannot fully retract it.
However, I have never experienced any discomfort throughout my life.
During a health check in my first year of high school, the doctor mentioned that my foreskin is too long and suggested circumcision.
Do you have any advice?

Hello: Your condition should not require surgery according to your examining physician; however, it is medically recommended.
The inability to retract the foreskin freely during an erection is an indication for surgery.
Wishing you good health.

More Info
Phimosis is a condition where the foreskin cannot be retracted over the glans (the head of the penis). It can be a common issue among males, especially in younger individuals, and can lead to concerns about hygiene, discomfort, or embarrassment, particularly in situations like communal bathing, as you mentioned.
In your case, it seems that you have a condition of phimosis that does not cause you any pain or discomfort, which is a positive aspect. However, the fact that you are unable to retract the foreskin fully, even when erect, is something that should be addressed, especially considering your upcoming transition to a public setting like a police academy.
Here are some potential solutions and considerations for managing phimosis:
1. Topical Steroid Creams: In some cases, applying a topical steroid cream prescribed by a healthcare provider can help to loosen the skin of the foreskin, making it easier to retract. This method is non-invasive and can be effective for mild cases of phimosis.
2. Gentle Stretching Exercises: If you are comfortable, you can try gently stretching the foreskin over time. This should be done carefully to avoid pain or injury. It’s important to ensure that your hands are clean to prevent any infections.
3. Consultation with a Urologist: Since your health check indicated that the foreskin is too long, it would be wise to consult a urologist. They can provide a thorough examination and discuss the best options for your specific situation. They may recommend a surgical procedure, such as circumcision or a preputioplasty, depending on the severity of the phimosis and your personal preferences.
4. Circumcision: This is a common surgical procedure where the foreskin is removed. It is often recommended for severe cases of phimosis, especially if it causes recurrent infections or other complications. While it is a permanent solution, many individuals find it beneficial in terms of hygiene and comfort.
5. Preputioplasty: This is a less invasive surgical option that involves widening the opening of the foreskin without complete removal. This can be a good alternative for those who wish to retain their foreskin but still need relief from phimosis.
6. Hygiene Practices: Regardless of the treatment you choose, maintaining good hygiene is crucial. If the foreskin cannot be retracted, it is important to clean the area gently to prevent any buildup of smegma, which can lead to infections.
7. Psychological Considerations: It’s understandable to feel anxious about this condition, especially with the social implications of attending a public facility. Remember that many individuals experience similar issues, and healthcare professionals are accustomed to discussing these matters with sensitivity and confidentiality.
In conclusion, while phimosis can be a source of concern, there are various options available to address it. Consulting with a healthcare professional, particularly a urologist, will provide you with tailored advice and treatment options. It’s important to address this issue not only for your physical health but also for your peace of mind as you prepare for your future endeavors.
