Prepuce issues
Hello, doctor.
I would like to ask about my condition.
The inner side of my foreskin is raw.
I went to see a doctor, and he said I have phimosis with inflammation, possibly caused by inadequate hygiene.
He prescribed me a topical ointment called Clindamycin, but after applying it for 4.5 days (twice daily), my symptoms have not improved, and the raw areas have increased.
Additionally, I would like to know if I should retract the glans after applying the ointment.
Won't that push the ointment out? Also, shortly after applying the ointment, I notice yellow discharge coming out.

Dr. Du Shixi reply Urology
Hello: It sounds like your issue is quite serious.
You need to keep the area clean and dry; it must not be moist.
After applying the ointment, it should be covered.
If it's confirmed to be pus, you may need to switch to a different antibiotic.
With the hot weather, many people experience mild inflammatory symptoms, but if there is any breaking of the skin, you must be cautious, as this area is difficult to care for and healing can be slow.
Wishing you good health.

More Info
When it comes to foreskin issues, particularly in the context of phimosis (tight foreskin) and inflammation, it's essential to understand the underlying causes and appropriate treatment options. Based on your description, it seems you are experiencing symptoms related to inflammation of the foreskin, possibly due to phimosis or inadequate hygiene.
Understanding the Condition
1. Phimosis: This is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis). It can lead to inflammation, pain, and sometimes infection if not properly managed.
2. Balanitis: This is the inflammation of the glans, often associated with phimosis. Symptoms can include redness, swelling, and discharge, which may be yellowish or pus-like.
3. Hygiene: Poor hygiene can exacerbate these conditions, as smegma (a combination of dead skin cells and oils) can accumulate under the foreskin, leading to irritation and infection.
Treatment Options
1. Topical Treatments: The medication you mentioned, likely a corticosteroid cream, is commonly prescribed to reduce inflammation and help with the retraction of the foreskin. However, if you have been using it for 4-5 days without improvement, it may be time to reassess the treatment plan.
2. Hygiene Practices: It's crucial to maintain proper hygiene. Gently retracting the foreskin (if possible) during bathing and cleaning the area with mild soap and water can help prevent further irritation.
3. Follow-Up: If the symptoms persist or worsen (such as increased pain, swelling, or discharge), it is important to follow up with your healthcare provider. They may need to examine you again to determine if there is an underlying infection or if a different treatment approach is necessary.
Application of Medication
Regarding your question about applying the ointment and retracting the foreskin:
- After Application: It is generally advisable to allow the medication to absorb for a few minutes before retracting the foreskin. This helps ensure that the medication has time to act on the affected area.
- Discharge Concerns: If you notice yellow discharge after applying the ointment, it could indicate an infection. In such cases, you should contact your doctor for further evaluation. They may need to prescribe an antibiotic if there is a bacterial infection present.
When to Seek Further Help
If you continue to experience symptoms such as:
- Increased pain or swelling
- Persistent discharge
- Difficulty retracting the foreskin
- Signs of infection (fever, increased redness, or warmth)
It is crucial to seek further medical advice. In some cases, surgical options, such as circumcision or preputioplasty (a less invasive procedure), may be considered if conservative treatments fail.
Conclusion
In summary, while topical treatments can be effective for foreskin issues, persistent symptoms warrant further evaluation. Maintaining good hygiene and following your doctor's instructions regarding medication application is essential. If you have concerns about the effectiveness of your current treatment or notice any worsening symptoms, do not hesitate to reach out to your healthcare provider for a reassessment.
