Unexplained bleeding
Hello, doctor.
First, thank you for taking the time to respond despite your busy schedule.
I am currently being treated for polycystic ovary syndrome (PCOS).
A few weeks ago, I took medication for 13 days to help with endometrial hyperplasia, and since then, I haven't experienced any unusual symptoms.
Due to concerns about affecting the treatment, I haven't been intimate with my boyfriend for a long time.
However, two days ago, feeling that my condition was stable, I agreed to be intimate.
The experience was not rough, and there was sufficient foreplay; the only difference was that my boyfriend was overly excited after such a long time and accidentally ejaculated inside me.
That day, I felt some discomfort in my abdomen, and the next day, I noticed a slight pink discharge, which has now progressed to a significant amount of pink bleeding.
I used a 35 cm overnight pad, but it still overflowed, and I am experiencing abdominal pain.
I am still taking medication for PCOS, and my doctor mentioned that I have a progesterone deficiency.
I'm concerned whether this situation will affect my condition.

Unexplained bleeding in women, particularly those with Polycystic Ovary Syndrome (PCOS), can be a source of significant concern and confusion. Your situation, where you have been diagnosed with PCOS and are experiencing unusual bleeding after a period of sexual activity, raises several important points that need to be addressed.
Firstly, it is essential to understand that PCOS is a hormonal disorder that can lead to irregular menstrual cycles, anovulation (lack of ovulation), and various other symptoms, including excessive hair growth and weight gain. Women with PCOS often experience irregular or prolonged menstrual bleeding due to hormonal imbalances. In your case, the bleeding you are experiencing could be attributed to these hormonal fluctuations, especially since you mentioned that you have been undergoing treatment for endometrial hyperplasia (thickening of the uterine lining) and have a history of low progesterone levels.
The fact that you experienced a light pink discharge that progressed to heavier pink bleeding after sexual intercourse could indicate several possibilities. One common cause of post-coital bleeding is cervical or vaginal irritation, which can occur if there has been a lack of sexual activity for a while. The tissues may be more sensitive, leading to minor bleeding. Additionally, if there is any underlying inflammation or infection, this could also contribute to bleeding.
Another possibility is that the hormonal treatment you are undergoing for PCOS and endometrial hyperplasia may be influencing your uterine lining's stability. Hormonal therapies can sometimes lead to unexpected bleeding patterns, especially if there are fluctuations in hormone levels. If you have low progesterone, this can lead to a lack of support for the uterine lining, which may cause it to shed irregularly.
It's also crucial to consider the possibility of other underlying conditions. While your doctor has indicated that your endometrial lining appears normal, persistent or heavy bleeding warrants further investigation. Conditions such as endometriosis, fibroids, or even cervical issues could be contributing factors. It is advisable to discuss these concerns with your healthcare provider, who may recommend additional tests, such as a pelvic ultrasound or a hysteroscopy, to rule out any other potential causes of your bleeding.
In terms of your concern about whether this bleeding could affect your fertility, it is important to note that while PCOS can impact ovulation and fertility, the bleeding itself is not likely to directly affect your ability to conceive. However, managing your PCOS effectively is crucial for optimizing your reproductive health. This may include lifestyle modifications, such as maintaining a healthy weight, regular exercise, and possibly medication to regulate your menstrual cycle and improve ovulation.
If you continue to experience heavy bleeding or if the abdominal pain worsens, it is essential to seek medical attention promptly. Your healthcare provider may need to adjust your treatment plan or explore other options to manage your symptoms effectively.
In summary, while unexplained bleeding in women with PCOS can be concerning, it is often related to hormonal imbalances and may not necessarily indicate a severe underlying condition. However, persistent symptoms should always be evaluated by a healthcare professional to ensure appropriate management and care.
