Sexual dysfunction
I am 34 years old and have experienced a significant decline in sexual function over the past few years.
I do not smoke or drink alcohol, sleep a full 7 hours every night, have a normal BMI, and do not have any issues with hypertension, diabetes, or hyperlipidemia.
However, I currently have very little sexual desire and difficulty achieving an erection unless stimulated, and even then, my erections are not very firm.
Last week, I took a medication called "Meihao Ting" and experienced only a semi-erection.
This issue has persisted for about two years.
I have ruled out psychological causes and have had my testosterone levels checked; one test was normal and another was even high.
I also underwent penile Doppler ultrasound, which showed a blood flow velocity of 29 cm/sec on the left and 20 cm/sec on the right.
The doctor indicated there is a problem but only recommended taking a low dose of sildenafil for a period of time.
I feel this approach only addresses the symptoms and not the underlying issue.
I am considering scheduling an appointment for low-intensity extracorporeal shockwave therapy.
Given that I have vascular issues, I am wondering if there could be other hormonal problems aside from testosterone.
What additional tests should I undergo? This issue is very distressing for me, and I sincerely seek your advice.
Thank you.

Regarding libido, if male hormone levels are normal, it is often related to the negative effects of work or life stress.
As for erectile function, low-intensity shockwave therapy often needs to be combined with the use of medications like Cialis, so this treatment approach is indeed appropriate!

More Info
Erectile dysfunction (ED) is a complex condition that can stem from various causes, including psychological, hormonal, and vascular factors. Given your age and the symptoms you've described, it's essential to take a comprehensive approach to understand the underlying issues contributing to your ED.
Causes of Erectile Dysfunction
1. Vascular Issues: As indicated by your Doppler ultrasound results showing reduced blood flow in the penis, vascular problems can significantly impact erectile function. The blood flow is crucial for achieving and maintaining an erection, and any impairment can lead to difficulties.
2. Hormonal Factors: While you mentioned that testosterone levels were checked and one test showed high levels, it's important to consider other hormones that can influence sexual function. For example, prolactin, luteinizing hormone (LH), and follicle-stimulating hormone (FSH) can also play a role in sexual desire and erectile function. An imbalance in these hormones could contribute to your symptoms.
3. Psychological Factors: Although you have ruled out psychological causes, it's worth noting that stress, anxiety, and depression can still play a role in ED. These factors can affect libido and the ability to achieve an erection, even if they are not the primary cause.
4. Lifestyle Factors: While you maintain a healthy lifestyle by not smoking or drinking and having a normal BMI, other factors such as lack of physical activity, poor diet, or sleep disturbances can also contribute to ED.
Recommended Tests
Given your situation, here are some tests you might consider discussing with your healthcare provider:
1. Hormonal Panel: In addition to testosterone, request tests for prolactin, LH, FSH, and estradiol. This will help assess whether hormonal imbalances are contributing to your symptoms.
2. Comprehensive Metabolic Panel: This can help evaluate your overall health and identify any underlying conditions that might affect erectile function.
3. Cardiovascular Assessment: Since you have vascular issues, a thorough cardiovascular evaluation may be beneficial. This could include an echocardiogram or stress testing to assess heart health.
4. Psychological Evaluation: If you haven't already, consider a consultation with a mental health professional who specializes in sexual health. They can help identify any underlying psychological factors that may not be immediately apparent.
Treatment Options
1. Medications: While you mentioned being prescribed a low dose of sildenafil (commonly known as Viagra), it is indeed a common first-line treatment for ED. However, it primarily addresses the symptoms rather than the underlying causes.
2. Low-Intensity Shockwave Therapy: This treatment aims to improve blood flow to the penis and may help with vascular issues. It is a non-invasive option that has shown promise in some studies.
3. Lifestyle Modifications: Continue to maintain a healthy lifestyle, and consider incorporating regular exercise, which can improve cardiovascular health and potentially enhance erectile function.
4. Counseling: If psychological factors are suspected, therapy can be beneficial. Cognitive-behavioral therapy (CBT) has been effective for many men experiencing ED.
5. Vacuum Erection Devices (VED): These devices can help draw blood into the penis and may be a useful adjunct to other treatments.
Conclusion
Erectile dysfunction can be multifactorial, and addressing it often requires a comprehensive approach. Given your symptoms and the vascular issues identified, it would be prudent to pursue further hormonal evaluations and consider additional treatment options beyond medication. Engaging with a urologist or an endocrinologist specializing in sexual health can provide you with tailored advice and treatment strategies. Remember, you are not alone in this, and many men experience similar challenges. Seeking help is the first step toward finding a solution.
