Eustachian tube dysfunction
I have read a lot about Eustachian tube dysfunction, but I still haven't found an answer.
I would like to consult a physician.
Recently, I may have been overthinking, and I feel a lot of pressure in my head.
I have difficulty concentrating when looking at things.
At night, when I lie down, my head feels heavy and swollen.
I toss and turn with my eyes closed and can't fall asleep.
Additionally, my right ear constantly picks up surrounding sounds, and they seem unusually clear.
Every time I swallow or yawn, I hear a "pop" sound.
Whether the wind is blowing towards my ear or it's calm, my right ear feels very open, and it feels like cold air is rushing in.
This has been very distressing for me.
Could you please explain what might be happening? Thank you.

Dr. Li Jiaxuan reply Otolaryngology
Your symptoms (the right ear involuntarily hears surrounding sounds very clearly, and you hear a "pop" sound every time you swallow or yawn; regardless of whether the wind is blowing towards the ear or if it's still, the right ear feels very open, and it feels like cold air is being pushed into it) are likely related to Eustachian tube dysfunction.
When you experience symptoms, it would be advisable to visit an otolaryngology clinic for an ear pressure test, as this should help assess the Eustachian tube's ability to equalize pressure.
Recent changes in atmospheric pressure may make your symptoms less likely to improve.
If you do not see improvement within two weeks, please consult an otolaryngologist for treatment.
Thank you!

More Info
Eustachian tube dysfunction (ETD) can lead to a variety of symptoms that can significantly affect your quality of life. The Eustachian tube connects the middle ear to the back of the throat and helps to equalize pressure, drain fluid, and protect the ear from pathogens. When this tube does not function properly, it can result in a range of issues, including the symptoms you are experiencing.
Symptoms of Eustachian Tube Dysfunction
1. Ear Pressure and Fullness: You mentioned feeling pressure in your brain and a sense of heaviness when lying down. This sensation is often associated with ETD, as the inability to equalize pressure can create discomfort in the ears and even lead to a feeling of fullness or pressure in the head.
2. Auditory Disturbances: The abnormal clarity of sounds you are experiencing in your right ear, along with the popping sounds when swallowing or yawning, are classic signs of ETD. This occurs because the Eustachian tube is not opening properly to equalize pressure, leading to a sensation of fullness and altered hearing.
3. Difficulty Concentrating: The pressure and discomfort can lead to headaches or a feeling of mental fog, which may explain your difficulty concentrating. This can be exacerbated by stress or anxiety, which often accompany physical discomfort.
4. Sleep Disturbances: The inability to find a comfortable position due to the pressure in your head can lead to insomnia or disrupted sleep patterns. This is not uncommon in individuals with ETD, as the discomfort can be more pronounced when lying down.
Potential Causes of Eustachian Tube Dysfunction
- Allergies and Sinus Issues: Allergic reactions can cause inflammation and swelling in the nasal passages and Eustachian tubes, leading to dysfunction. Sinus infections can also contribute to this problem.
- Upper Respiratory Infections: Colds or other respiratory infections can lead to mucus buildup and swelling, blocking the Eustachian tube.
- Changes in Altitude: Rapid changes in altitude, such as flying or driving in mountainous areas, can affect the pressure in the ears.
- Anatomical Issues: Some individuals may have structural issues with their Eustachian tubes that predispose them to dysfunction.
Solutions and Management Strategies
1. Nasal Decongestants: Over-the-counter decongestants can help reduce swelling in the nasal passages and Eustachian tubes, making it easier for them to function properly.
2. Nasal Steroid Sprays: These can help reduce inflammation in the nasal passages and Eustachian tubes, particularly if allergies are a contributing factor.
3. Swallowing Techniques: Engaging in swallowing or yawning can help open the Eustachian tubes. Chewing gum or sucking on candy can also stimulate swallowing.
4. Valsalva Maneuver: This technique involves closing your mouth, pinching your nose shut, and gently blowing to equalize pressure in the ears. However, this should be done cautiously and not excessively.
5. Avoiding Irritants: If allergies are a factor, avoiding known allergens and irritants can help reduce symptoms.
6. Consultation with an ENT Specialist: If symptoms persist, it may be beneficial to see an ear, nose, and throat (ENT) specialist. They can evaluate your condition more thoroughly and may suggest treatments such as balloon dilation of the Eustachian tube or other interventions.
Conclusion
Your symptoms of pressure, auditory disturbances, and sleep issues are consistent with Eustachian tube dysfunction. It is essential to address these symptoms, as they can significantly impact your daily life. Implementing some of the management strategies mentioned above may provide relief, but if symptoms persist or worsen, seeking professional medical advice is crucial. An ENT specialist can offer targeted treatments to help restore normal function to your Eustachian tubes and alleviate your discomfort.
