Questions After a Miscarriage

My girlfriend and I chose to undergo a medical abortion around May 17 due to personal circumstances.
After the miscarriage, she occasionally stayed up late and had poor sleep quality.
During this time, we engaged in normal sexual activity with protection until June 10, when we decided to try in vitro fertilization (IVF) after the protection was no longer available.
Recently, she has been feeling unwell, experiencing fever and vomiting, and on June 24, she went to the emergency room.
During a urine test, it seemed that she tested positive for pregnancy, but an ultrasound did not show any embryos yet.

I would like to ask if there is a chance of her becoming pregnant again after external ejaculation (confirming that it was outside).
Additionally, after a miscarriage, does ovulation still occur before the next menstrual period? I have been searching for information online and have been somewhat misled.
Also, before the next menstrual period, could pregnancy test results still show signs of pregnancy due to hormonal influences? What is the correct timing for a pregnancy test to be accurate? Should it be before or after the next menstrual period? Lastly, how long after a miscarriage does it typically take for menstruation to return to normal?

Lu, 30~39 year old female. Ask Date: 2018/06/25

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
Ovulation can occur just before the menstrual period.
It is recommended to see a doctor for a pregnancy test in a week to confirm.
Thank you! Wishing you peace and health, Dr.
Zhang.

After experiencing a miscarriage, many couples have questions regarding fertility, ovulation, and the timing of pregnancy tests. Your concerns are valid, and it's essential to understand the physiological changes that occur after a miscarriage, as well as the implications for future pregnancies.

Firstly, regarding the possibility of pregnancy after a miscarriage, it is indeed possible to conceive before your girlfriend's next menstrual period. After a miscarriage, the body can return to its normal ovulation cycle relatively quickly, often within two to four weeks. This means that ovulation can occur before the next expected period, allowing for the possibility of conception if unprotected intercourse occurs during this time. However, the timing can vary significantly from person to person, depending on individual hormonal levels and overall health.

In your case, since you mentioned that you engaged in protected intercourse until June 10 and then opted for in vitro fertilization (IVF), it is crucial to clarify that IVF typically involves the retrieval of eggs and fertilization outside the body, which is a different process than natural conception. If you are asking about the likelihood of natural conception occurring after the miscarriage and before the IVF procedure, it is possible, especially if ovulation occurred during that time.

Regarding the pregnancy test results, it is important to note that after a miscarriage, the hormone hCG (human chorionic gonadotropin) can remain in the body for a period of time. This means that a urine pregnancy test may still show a positive result even if a new pregnancy has not occurred. Typically, hCG levels should return to zero within a few weeks after a miscarriage, but this can vary. Therefore, if your girlfriend is experiencing symptoms such as fever and vomiting, it is essential to consult a healthcare provider to rule out any complications or confirm a new pregnancy.

As for the timing of pregnancy tests, the most accurate results are usually obtained about one week after a missed period. If your girlfriend's menstrual cycle is regular, this would be the best time to test. Testing too early can lead to false negatives, especially if hCG levels are still fluctuating due to the recent miscarriage.

In terms of menstrual cycle normalization, many women find that their cycles return to normal within one to three months after a miscarriage. However, this can vary widely. Some may experience irregular cycles initially, while others may find that their cycles return to their pre-pregnancy patterns quickly.

In summary, it is possible to conceive after a miscarriage before the next menstrual period, and ovulation can occur within weeks post-miscarriage. Pregnancy tests should ideally be taken about a week after a missed period for the most accurate results. If there are any concerns about symptoms or the possibility of complications, it is always best to seek medical advice from a healthcare professional. They can provide personalized guidance based on your girlfriend's health history and current situation.
