Root canal treatment
On September 6th, I had my first root canal treatment.
The dentist noticed vertical cracks in the tooth and hoped to save it.
On the 22nd, during the third treatment, I am now waiting for the crown.
Starting on the 26th, I experienced gum swelling and pain, with a large abscess.
After drainage of the pus, the swelling persisted, and I am currently undergoing antibiotic treatment.
I would like to know what caused the gum swelling and whether the tooth needs to be extracted.

If there is a suspicion of a vertical tooth fracture that extends to the root, even after undergoing root canal treatment, there may still be gum swelling and pain, or even the development of abscesses.
It is advisable to consult a dentist for further evaluation.

More Info
Gum swelling after root canal treatment can be a concerning symptom, and understanding its causes and potential solutions is crucial for effective management. Based on your description, it appears that you have undergone multiple sessions of root canal treatment, and now you are experiencing gum swelling accompanied by a significant abscess. This situation can arise from several factors related to the root canal procedure and the condition of the tooth prior to treatment.
Causes of Gum Swelling After Root Canal Treatment
1. Infection: One of the most common reasons for gum swelling post-root canal treatment is the presence of infection. If bacteria remain in the root canal system, they can lead to inflammation and the formation of an abscess, which is a localized collection of pus. This can cause swelling in the surrounding gum tissue.
2. Incomplete Cleaning: During root canal therapy, the dentist aims to clean and disinfect the entire root canal system. However, if there are additional canals that were not identified or adequately treated, or if the canals were blocked, this can lead to persistent infection and subsequent swelling.
3. Tissue Reaction: Sometimes, the body may react to the materials used during the root canal treatment, such as the filling material or the temporary crown. This reaction can manifest as inflammation and swelling in the gums.
4. Post-Operative Complications: After a root canal, it is not uncommon for patients to experience some discomfort or swelling as part of the healing process. However, if the swelling persists or worsens, it may indicate a complication that requires further evaluation.
5. Abscess Formation: The presence of a large abscess, as you mentioned, suggests that there is a significant infection that may need to be drained. An abscess can cause considerable pain and swelling and may require immediate attention.
Solutions and Recommendations
1. Immediate Dental Evaluation: Given that you have developed a large abscess and are experiencing significant swelling, it is essential to seek immediate dental care. Delaying treatment can lead to further complications, including the risk of systemic infection.
2. Antibiotic Treatment: Since you are currently on antibiotics, it is important to complete the course as prescribed. Antibiotics can help control the infection, but they may not resolve the underlying issue if the source of the infection is not addressed.
3. Drainage of Abscess: If the abscess is large and causing significant discomfort, your dentist may need to perform a procedure to drain the pus. This can provide immediate relief and help reduce swelling.
4. Further Imaging: Your dentist may recommend additional imaging, such as X-rays or a cone beam CT scan, to assess the extent of the infection and to check for any missed canals or other complications.
5. Consideration of Tooth Extraction: If the tooth is severely compromised, or if the infection cannot be adequately managed through root canal therapy, extraction may be necessary. Your dentist will evaluate the condition of the tooth and discuss the best course of action with you.
6. Follow-Up Care: After addressing the immediate concerns, follow-up appointments will be crucial to monitor healing and to determine the next steps, whether that involves further treatment of the tooth or planning for a replacement option, such as an implant.
In conclusion, gum swelling after root canal treatment can be attributed to infection, incomplete cleaning, or tissue reactions. Given your symptoms, it is imperative to seek prompt dental care to address the abscess and prevent further complications. Your dentist will be able to provide a tailored treatment plan based on a thorough evaluation of your condition.
