Erectile dysfunction issues?
I would like to ask the doctor if varicocele can affect penile rigidity and erection.
Recently, I have noticed that my penis does not become erect without stimulation.
However, with continuous stimulation, it can remain erect, but it softens within a minute without stimulation.
I do experience morning erections, but when I wake up, they don't feel very firm.
Interestingly, on days when I don't sleep well and feel refreshed, I wake up early and have particularly firm morning erections.
I can also achieve an erection by viewing pornographic images.
Additionally, I have a moderate mitral valve regurgitation issue; could this have an impact? I have been under significant mental stress for various reasons for a long time.

Dr. Du Shixi reply Urology
Hello: Varicocele can affect penile rigidity and erection.
However, individual penile hardness is related to personal constitution and physical condition; it is normal for it to vary at times.
Moderate mitral valve regurgitation should not have a significant impact.
It is important to pay attention to overall health, maintain a balanced diet, and engage in moderate exercise, as these can be beneficial.
Wishing you good health.

Varicocele, a condition characterized by the enlargement of veins within the scrotum, is often compared to varicose veins in the legs. It is a common condition that can affect male fertility and has been linked to various sexual dysfunctions, including erectile dysfunction (ED). Understanding the connection between varicocele and erectile function is crucial for men experiencing symptoms like the ones you described.
Research indicates that varicocele can potentially impact erectile function through several mechanisms. One of the primary ways is through hormonal imbalance. Varicocele can lead to increased levels of estrogen and decreased levels of testosterone in the body. Testosterone is essential for sexual arousal and maintaining erectile function. A decrease in testosterone levels can lead to difficulties in achieving and maintaining an erection.
Additionally, varicocele can cause testicular hypoxia (lack of oxygen) and increased scrotal temperature, which can adversely affect sperm production and overall testicular health. This can lead to a cascade of physiological changes that may also affect erectile function. The relationship between the testicles and erectile function is complex, as the testicles produce hormones that are vital for sexual health.
In your case, the symptoms you describe—difficulty achieving an erection without stimulation, variations in the firmness of morning erections, and the influence of mental state on erectile quality—could be influenced by both psychological and physiological factors. Psychological factors, such as stress and anxiety, can significantly impact erectile function. Given that you mentioned experiencing significant mental stress, it is possible that this is contributing to your erectile difficulties. Stress can lead to increased levels of cortisol, which can negatively affect testosterone levels and sexual function.
Moreover, your existing heart condition (mitral valve regurgitation) could also play a role in your erectile function. Cardiovascular health is closely linked to erectile function, as erections require adequate blood flow. If your heart condition affects your overall cardiovascular health, it may also impact your ability to achieve and maintain an erection.
It is important to consider that while varicocele may contribute to erectile dysfunction, it is not the sole factor. Other potential causes include psychological issues, hormonal imbalances, and cardiovascular health. Since you mentioned that you can achieve an erection with stimulation and that your morning erections vary, it suggests that there may be a psychological component at play, particularly if you experience fluctuations based on your mental state.
If you are concerned about the impact of varicocele on your erectile function, it is advisable to consult with a healthcare professional. They can perform a thorough evaluation, which may include hormone level testing, a physical examination, and possibly imaging studies to assess the varicocele. Treatment options for varicocele, such as surgical intervention, may improve both fertility and erectile function in some cases.
In summary, varicocele can affect erectile function through hormonal changes and physiological impacts on testicular health. However, psychological factors and existing health conditions also play a significant role. Addressing these concerns with a healthcare provider can help you find the most appropriate treatment and improve your overall sexual health.
