Bowel issues
Doctor, I apologize for disturbing you.
I am currently a high school junior transitioning to senior year.
For a long time, my bowel movements have been dark and unformed.
I used to not think much of it, but in recent months, I have often felt fatigued, even with sufficient sleep.
My meals are regular and normal, yet my weight has not increased; I feel like I am losing weight.
Even more alarming, a few days ago, after using the restroom and flushing, I noticed small red-black worms (possibly moving) in the toilet.
I couldn't believe it at first, but I have seen them several times after using the bathroom.
With important exams approaching, I am very worried.
What should I do, doctor?

Dear Student Ming,
Hello.
You have reported abnormal bowel movements, often with dark-colored and unformed stools, and a feeling of weight loss.
Recently, there have been red and black substances in your stool, and you are feeling anxious.
The color of stool is related to the food intake; iron-rich foods can lead to darker stools, while consuming red foods like dragon fruit and hibiscus can result in a reddish-black stool color.
The unformed nature of your stool may be associated with a sensitive bowel condition, such as irritable bowel syndrome, which can be influenced by your mood and diet.
I recommend a stool examination to check for the presence of parasites, eggs, and signs of inflammation or bleeding.
If eggs are found, a typical deworming medication can be effective within three to five days.
Please pay attention to dietary hygiene and maintain a positive mood to support your intestinal health.
Wishing you peace, health, and well-being.
Sincerely,
Dr.

Dear Patient,
Thank you for reaching out with your concerns. It’s important to address the symptoms you are experiencing, as they could indicate underlying health issues that require medical attention.
Firstly, the presence of black stools, especially when accompanied by fatigue and weight loss, is a significant symptom that should not be ignored. Black stools can be a sign of gastrointestinal bleeding, often due to conditions such as peptic ulcers, gastritis, or even more serious issues like tumors. The fact that you have also noticed what appears to be small red-black worms in your stool is particularly concerning. This could suggest a parasitic infection, which can lead to malabsorption of nutrients, further contributing to your weight loss and fatigue.
Given your symptoms, I strongly recommend that you seek medical attention as soon as possible. A healthcare professional will likely perform a thorough evaluation, which may include a physical examination, blood tests, and possibly imaging studies or endoscopy to determine the cause of your symptoms. It’s crucial to rule out any serious conditions, especially since you are experiencing fatigue and unintentional weight loss.
In addition to the gastrointestinal symptoms, it’s important to consider your overall health and lifestyle. Ensure that you are maintaining a balanced diet rich in nutrients, as this can help support your body during times of stress, such as preparing for exams. Staying hydrated and getting adequate sleep are also essential for your overall well-being.
If you are diagnosed with a parasitic infection or any gastrointestinal issue, treatment will vary depending on the specific diagnosis. For instance, if it is a parasitic infection, antiparasitic medications will be prescribed. If there is evidence of bleeding, further interventions may be necessary to address the source of the bleeding.
Lastly, while it’s understandable to feel anxious about your health, especially with exams approaching, it’s vital to prioritize your health first. Stress can exacerbate physical symptoms, so finding healthy coping mechanisms, such as mindfulness or relaxation techniques, can be beneficial.
In summary, please seek medical attention promptly to address your symptoms. Early diagnosis and treatment are key to managing any potential health issues effectively. Take care of yourself, and I wish you the best in your health and studies.
Best regards.
