Erectile function

Hello Doctor: I would like to introduce myself.
I am 23 years old, do not smoke or drink alcohol, do not consume much meat or fish, and do not drink bubble tea.
I have a regular sleep schedule, going to bed at 11 PM and waking up at 7 AM.
I exercise more than three times a week, and I have no stress related to my lifestyle, finances, or relationships.
I have mitral valve prolapse and experience almost no morning erections.
I have two ongoing issues that I would like to discuss with you:
1.
After achieving an erection, I can reach normal hardness, but I cannot maintain blood within the blood vessels.
As soon as there is no exertion or continuous stimulation, it quickly becomes soft again.
I am unable to sustain the engorged state.
Normally, after an erection, blood is retained, and even when relaxed, the erection remains firm.
However, it seems that the "locking" mechanism in my case is malfunctioning.
2.
After an erection, there is usually a very noticeable, tight, and swollen sensation, where the entire penis feels full of strength.
However, I currently cannot feel the presence of my penis after an erection; it feels as if there are no nerves, and it has completely disappeared.
I have searched online but found no relevant cases to reference.
I appreciate your assistance with these two issues.
Thank you.

If there is a history of trauma, it is recommended to undergo cavernosography.
Online consultations cannot replace an in-person evaluation and physical examination by a physician.
If there is a medical condition, it is still advised to seek care at a medical facility.

Erectile dysfunction (ED) is a condition that can affect men of all ages, including young men like yourself. Your experience and concerns are valid, and it's important to address them with a comprehensive understanding of the underlying factors that may contribute to your symptoms.

Firstly, let's break down the two main issues you've described. The first concern is the inability to maintain an erection after achieving initial hardness. This phenomenon can be attributed to several physiological and psychological factors. In a healthy erection, blood flows into the penis, filling the erectile tissues and causing it to become firm. This process is maintained by a combination of blood flow, nerve signals, and the constriction of veins that prevent blood from flowing back out. If you find that your erection softens quickly after stimulation or without sustained effort, it may indicate an issue with the vascular system or the nerve signals that are necessary to maintain that erection.

Several potential causes could be at play here. For instance, anxiety or performance pressure can significantly impact erectile function, even in young men. Additionally, conditions such as hormonal imbalances, particularly low testosterone levels, can also affect your ability to maintain an erection. Given your age and healthy lifestyle, it may be less likely that these are the primary causes, but they should still be considered.

The second concern you mentioned is the lack of sensation in the penis during an erection. This could be indicative of nerve-related issues. The nerves that supply the penis are crucial for both the sensation and the physiological processes involved in achieving and maintaining an erection. If these nerves are not functioning properly, it could lead to a diminished sense of fullness or pleasure during an erection. Conditions such as diabetes, pelvic injuries, or even certain medications can affect nerve function.
Given that you have a history of mitral valve prolapse, it's also worth considering whether any medications you might be taking for this condition could have side effects that impact erectile function. Some medications can lead to decreased libido or erectile difficulties.

It's important to approach this situation holistically. Here are some steps you can take:
1. Consult a Specialist: Since you are experiencing these symptoms, it would be beneficial to consult a urologist or an endocrinologist. They can perform a thorough examination, including blood tests to check hormone levels, and assess vascular health.

2. Psychological Evaluation: If anxiety or psychological factors are suspected, a consultation with a mental health professional may be helpful. Cognitive-behavioral therapy (CBT) can be effective in addressing performance anxiety and other psychological barriers to sexual function.

3. Lifestyle Factors: While you maintain a healthy lifestyle, consider evaluating all aspects of your health, including sleep quality, stress levels, and any potential underlying health conditions that may not be immediately apparent.

4. Open Communication: If you are in a relationship, discussing your concerns with your partner can alleviate some of the pressure you may feel during intimate moments. Open communication can foster understanding and support.

5. Follow-Up: Keep track of your symptoms and any changes you notice over time. This information can be valuable for your healthcare provider in diagnosing and treating your condition.

In conclusion, erectile dysfunction at a young age can be distressing, but it is important to remember that it is a common issue and can often be treated effectively. By seeking professional help and addressing both the physical and psychological aspects of your symptoms, you can work towards regaining confidence and improving your sexual health.
