Penile insecurity?

1.
I feel very self-conscious about my penis, which is slender and long.
I need a condom with a width of 46mm.
I wonder if it will grow longer or thicker at my age.

2.
My foreskin is quite long; when not erect, it completely covers the glans, and there is a fold of foreskin in front of the urethra (about 1cm in length).
Visually, it appears to be phimosis.
Even when erect, the foreskin still covers more than three-quarters of the glans.
I can retract it painlessly, but during intercourse, the foreskin tends to slide forward and cover the glans.
When urinating, I generally do not have an erection, so urine often gets on the foreskin, leading to smegma buildup daily, and there is an odor even after washing.
I've heard many people say that I have a long foreskin and that circumcision is not necessary, as it may result in loss of sensitivity.
Given my situation, would you recommend circumcision or not?
3.
I would like to know the correct method for measuring the penis.
For length, should I use a ruler from the glans (excluding the foreskin) to the pubic bone (not sure if it's the pubic bone, but it's the lowest bone pressed above the penis)? For girth, should I measure with a tape measure at the glans, the midpoint of the shaft, and the base of the penis (not pressing against the pubic bone, just visually at the base) and take the average? Is this the medical standard for measurement, or is there no defined method?
4.
There are many methods and surgeries advertised for penis enlargement: (1) Which is better among circumcision, laser circumcision, and dorsal slit surgery? Could you explain the pros and cons of each method? I would also appreciate recommendations for surgical options.
(2) Is it possible to train to lengthen or thicken the penis? I've seen articles online, including "The Complete Guide to Natural Penis Enlargement Exercises," are these methods safe and effective?
5.
There are many articles about training the pelvic floor muscles (PC muscles).
Do these actually exist, and can they help maintain a strong and firm penis, or is this just an internet myth?
6.
What is the normal frequency of masturbation that is considered healthy?

Hello: 1) The chances are slim.
2) Given your situation, surgery is not necessary, and you may be even less satisfied with the size after surgery.
3) There is a definition, which is similar to what you mentioned.
4) 1) If you can avoid surgery, then you don’t need it; if surgery is necessary, all three options are viable, and there is no recommended surgical method.
2) Don’t attempt it; if you get injured, you might regret it.
5) You can live normally with moderate exercise; if effective, the results will be minimal.
6) There is no standard for frequency; as long as it does not interfere with your life, work, or studies, it is acceptable.
Your body will let you know if you are overdoing it.
Wishing you good health.

When it comes to concerns about penis size and related anxieties, it’s important to approach the topic with a combination of medical knowledge and psychological understanding. Let’s break down your questions and concerns systematically.

1. Self-Perception and Size Anxiety: It's common for individuals to feel self-conscious about their penis size, especially in a society that often emphasizes certain ideals. The average erect penis size is typically around 5 to 6 inches in length and about 4.5 to 5 inches in circumference. If you are using a condom with a width of 46mm, it suggests that you may be on the smaller side of the average spectrum. However, it’s crucial to understand that size does not determine sexual satisfaction for you or your partner. As for growth, most males reach their full size by the late teens to early twenties, so significant changes in size after this period are unlikely.

2. Long Foreskin Concerns: The condition you describe, where the foreskin covers the glans (head of the penis) significantly, is known as phimosis. If you can retract the foreskin without pain, it may not be necessary to undergo circumcision. However, if you experience discomfort during sexual activity or hygiene issues (like the buildup of smegma), discussing circumcision with a urologist may be beneficial. Circumcision can reduce the risk of infections and improve hygiene, but it’s essential to weigh the potential loss of sensitivity against the benefits.

3. Measuring Penis Size: The standard method for measuring penis length is to use a ruler from the tip of the glans to the pubic bone, pressing down slightly to account for any fat pad. For girth, measuring at the thickest part of the shaft with a flexible measuring tape is recommended. It’s important to take multiple measurements to get an accurate average, as size can vary slightly due to factors like temperature and arousal.

4. Surgical Options and Effectiveness: There are various surgical methods for penis enlargement, including circumcision, which can sometimes be performed to address phimosis, and other procedures like penile lengthening or girth enhancement surgeries. Each method has its pros and cons:
- Circumcision: Can improve hygiene and reduce the risk of certain infections. However, it may lead to decreased sensitivity for some men.

- Penile Lengthening: Involves cutting the suspensory ligament, which can increase length but may lead to complications like scarring or erectile dysfunction.

- Girth Enhancement: Often involves fat grafting or dermal fillers, which can provide temporary results but carry risks of unevenness or complications.

5. Non-Surgical Methods: There are claims about exercises or devices that can increase size, such as vacuum pumps or stretching techniques. While some may see temporary results, these methods can be risky and may lead to injury if not done correctly. Always consult with a healthcare professional before trying any such methods.

6. Pelvic Floor Exercises: Strengthening the pelvic floor muscles (often referred to as Kegel exercises) can improve sexual function and may enhance erections. These exercises are safe and can be beneficial for overall sexual health.

7. Masturbation Frequency: There is no "normal" frequency for masturbation; it varies widely among individuals. As long as it does not interfere with daily life or responsibilities, it is generally considered a healthy practice.

In conclusion, it’s essential to address both the physical and psychological aspects of your concerns. Consulting with a urologist can provide personalized advice and treatment options. Additionally, speaking with a mental health professional can help you work through feelings of inadequacy and anxiety related to body image. Remember, sexual health encompasses much more than size; communication, intimacy, and mutual satisfaction are key components of a fulfilling sexual experience.
