Chronic prostatitis
Hello doctor, I have been suffering from chronic non-bacterial prostatitis for about five years.
In the first year, I experienced discomfort in the perineal area, but as that improved, I stopped taking medication.
However, over the past few years, I still have to wait for a while to urinate, and sometimes during winter, urination is accompanied by sharp pain in the prostate.
Recently, I've started to feel swelling and a burning sensation in the perineal area, which sometimes extends to my testicles, causing discomfort that feels better when I spread my legs.
Additionally, after ejaculation, it takes longer than before for my penis to return to its normal size.
Therefore, I went back to see the doctor.
The digital examination was painful, but the fluid that was expressed was still sterile.
However, a subsequent urinalysis revealed hematuria (not visible to the naked eye).
The doctor suggested a CT scan, but I am hesitant due to the high radiation exposure.
Given my young age, is it possible for me to have prostate cancer, or has my prostatitis worsened? I used to masturbate once a day, occasionally two to three times.
Could this be related? Thank you.

Hello: Chronic non-bacterial prostatitis is indeed very difficult to treat.
I have patients who have experienced intermittent issues for decades.
You may need to not only take medication but also pay attention to lifestyle factors.

More Info
Chronic prostatitis, particularly the non-bacterial type, can be a challenging condition to manage, especially when symptoms persist despite various treatments. Your description indicates a long history of discomfort and evolving symptoms, which can be quite distressing. Let's break down your concerns regarding chronic prostatitis, its symptoms, potential risks, and treatment options.
Understanding Chronic Prostatitis
Chronic prostatitis is classified into several categories, with the non-bacterial type being the most common. Symptoms can include pelvic pain, urinary difficulties, and sexual dysfunction. In your case, the discomfort in the perineum, the sensation of swelling, and the pain radiating to the testicles are all consistent with chronic pelvic pain syndrome (CPPS), which is often categorized under non-bacterial prostatitis.
Symptoms and Risks
The symptoms you've described, such as the burning sensation during urination, the feeling of incomplete bladder emptying, and the discomfort during ejaculation, are common in chronic prostatitis. The presence of blood in the urine (hematuria), even if not visible to the naked eye, warrants further investigation. While chronic prostatitis itself is not directly linked to prostate cancer, persistent symptoms and changes in urinary habits should be evaluated to rule out other underlying conditions, including cancer.
Treatment Options
1. Medications: Since you've already tried various medications, including antibiotics and pain relievers, it might be worth discussing alternative options with your healthcare provider. Alpha-blockers, which relax the muscles around the bladder neck and prostate, can sometimes help alleviate urinary symptoms. Additionally, medications like anti-inflammatory drugs or muscle relaxants may provide relief.
2. Physical Therapy: Pelvic floor physical therapy can be beneficial for managing chronic pelvic pain. A trained therapist can help you learn exercises to relax and strengthen the pelvic floor muscles, which may alleviate some of your symptoms.
3. Lifestyle Modifications: Maintaining a healthy lifestyle can significantly impact your symptoms. Regular exercise, a balanced diet, and avoiding irritants such as caffeine, alcohol, and spicy foods can help reduce flare-ups. Stress management techniques, including mindfulness and relaxation exercises, can also be beneficial.
4. Heat Therapy: Warm baths or heating pads applied to the pelvic area can provide temporary relief from pain and discomfort.
5. Surgery: In rare cases, if conservative treatments fail and symptoms are severe, surgical options may be considered. However, this is typically a last resort.
Concerns About Prostate Cancer
Your concern about prostate cancer is understandable, especially given your symptoms and the presence of blood in your urine. While prostate cancer is less common in younger individuals, it is not impossible. The best course of action is to follow your doctor's advice regarding imaging studies, such as a CT scan, to rule out any serious conditions. The risks associated with radiation exposure from a CT scan are generally outweighed by the benefits of diagnosing potential issues.
Sexual Activity and Its Impact
Regarding your question about masturbation, it is generally considered safe and can even be beneficial for prostate health. However, if you notice that frequent ejaculation exacerbates your symptoms, it may be worth moderating the frequency and observing any changes in your condition.
Conclusion
Chronic prostatitis can be a complex condition requiring a multifaceted approach to treatment. It is crucial to maintain open communication with your healthcare provider, discuss any new or worsening symptoms, and explore all available treatment options. Regular follow-ups and monitoring are essential to ensure that any potential complications are addressed promptly. Remember, while chronic prostatitis can be uncomfortable and frustrating, it is manageable with the right strategies and support.
