Coughing and chest tightness?
Hello, doctor; I am studying Animal Health at a vocational school.
Last September, when I first entered the rabbit barn on the farm, I started coughing after cleaning the barn.
I went to see an ENT specialist, and after taking medication for a month or two, there was no improvement.
Then I visited a pulmonologist and had an X-ray taken.
The doctor said my lungs were clear and prescribed me nasal medication, but after taking it, I felt very sleepy during class, and my symptoms did not improve.
I then consulted a traditional Chinese medicine practitioner.
In total, I have been on medication for half a year, but I still cough.
When I cough, I feel a vibration throughout my chest.
Once, after running on the track during physical education class, I felt short of breath and heard wheezing.
Which specialty should I see? I have been coughing for almost a year!

Hello: Your symptoms cannot rule out asthma.
I'm not sure if you had allergic rhinitis in your childhood.
I recommend that you see a pulmonologist and inform the doctor about the wheezing you experience after exercise.
Please ask the doctor to evaluate whether you have asthma!

Persistent cough and chest tightness can be quite distressing, especially when they linger for an extended period, as you have experienced. Given your history of exposure to a rabbit farm and the subsequent development of your symptoms, it is essential to consider a few potential underlying causes and the appropriate specialists to consult.
Firstly, your persistent cough could be attributed to several factors, including allergic reactions, respiratory infections, or even environmental irritants. Given that you have been in a farm setting, it is possible that you may have developed an allergy to animal dander or other allergens present in that environment. Allergic rhinitis or asthma could manifest as a chronic cough, especially if you are also experiencing chest tightness and wheezing.
Since you have already consulted an ENT specialist and a pulmonologist (chest physician) without significant improvement, it may be beneficial to consider seeing an allergist or immunologist. These specialists can conduct specific tests, such as skin prick tests or blood tests, to identify any allergies you may have developed. If allergies are confirmed, they can recommend appropriate treatments, including antihistamines, nasal corticosteroids, or even allergy shots (immunotherapy) to help manage your symptoms.
Additionally, your symptoms of chest tightness and wheezing, particularly after physical exertion, raise the possibility of asthma. Asthma is characterized by inflammation and narrowing of the airways, leading to difficulty breathing, coughing, and wheezing. A pulmonologist can perform pulmonary function tests (PFTs) to assess your lung function and determine if asthma is a contributing factor to your symptoms. If asthma is diagnosed, a tailored management plan involving inhalers (bronchodilators and corticosteroids) can significantly improve your quality of life.
Moreover, considering that you have been experiencing these symptoms for nearly a year, it may be prudent to rule out other potential causes, such as chronic bronchitis or even occupational lung diseases, given your exposure to a farm environment. A thorough evaluation by a pulmonologist, including imaging studies like a high-resolution CT scan of the chest, may be warranted to assess for any structural changes or chronic conditions affecting your lungs.
In summary, I recommend the following steps:
1. Consult an Allergist/Immunologist: To evaluate for potential allergies related to your environment and provide appropriate management.
2. Follow-up with a Pulmonologist: If you have not already undergone pulmonary function tests, this would be a crucial next step to assess for asthma or other lung conditions.
3. Consider Occupational Health Evaluation: Given your exposure to a farm environment, an occupational health specialist may provide insights into any work-related respiratory issues.
4. Keep a Symptom Diary: Documenting your symptoms, triggers, and any patterns can provide valuable information to your healthcare providers.
5. Lifestyle Modifications: In the meantime, consider avoiding known irritants, maintaining good indoor air quality, and practicing breathing exercises to help manage your symptoms.
Persistent cough and chest tightness can be complex, but with the right specialist and a comprehensive approach, you can find relief and improve your overall health. Don't hesitate to advocate for yourself and seek further evaluations until you find the answers you need.
