Diarrhea accompanied by bleeding?

Hello Doctor, I have recently changed jobs, and the stress and tension have led to diarrhea, occurring at least twice a day for the past five days.
Today, I suddenly noticed that my diarrhea (loose stools) contained quite a bit of fresh blood.
However, when I checked my anus with a mirror, I did not see any fissures or traces of blood.
I had a fecal occult blood test in May that was negative, but I still feel a bit anxious, especially since it has been 2-3 months.
I would like to ask if this situation or my age increases the likelihood of something serious.
Additionally, I have been diagnosed with internal hemorrhoids; I would like to know how a doctor would assess this if I cannot feel anything with my finger.
Thank you, Doctor.

Diarrhea with blood can be a concerning symptom, and it’s important to understand the potential causes and when to seek medical attention. In your case, the presence of fresh blood in your stool, especially after experiencing diarrhea for several days, warrants careful consideration.

Possible Causes of Blood in Diarrhea
1. Hemorrhoids: One of the most common causes of blood in the stool is hemorrhoids, which are swollen veins in the rectum or anus. They can cause bright red blood on the toilet paper or in the stool, especially during bowel movements. However, if you do not see any external signs or feel any lumps, it may not be the cause.

2. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard stools. They can lead to bleeding, but you mentioned not seeing any signs of fissures.

3. Gastrointestinal Infections: Bacterial infections (like those from E. coli or Salmonella) can cause diarrhea that may be bloody. These infections can be associated with other symptoms such as fever, abdominal pain, and nausea.

4. Inflammatory Bowel Disease (IBD): Conditions like Crohn's disease or ulcerative colitis can lead to bloody diarrhea. These are chronic conditions that often present with other symptoms such as abdominal pain, weight loss, and fatigue.

5. Diverticulosis: This condition involves the formation of small pouches in the colon wall, which can sometimes bleed.

6. Colorectal Cancer: Although less common in younger individuals, it is still a possibility, especially if there are other risk factors or family history.

When to Seek Medical Attention
Given that you have experienced diarrhea for five days and now have blood in your stool, it is advisable to seek medical attention promptly. A healthcare provider may perform a physical examination and possibly recommend tests such as:
- Stool Tests: To check for infections or blood.

- Colonoscopy: This procedure allows the doctor to view the inside of your colon and rectum, which can help identify any abnormalities.

- Imaging Studies: Such as a CT scan if there are concerns about more serious conditions.

Concerns Based on Age and Symptoms
While your age may reduce the likelihood of certain serious conditions, it does not eliminate the risk. The presence of blood in your stool, regardless of age, should be taken seriously. Your previous negative fecal occult blood test is reassuring, but it does not rule out new developments.

Conclusion
In summary, while there are several potential causes for your symptoms, it is crucial to consult with a healthcare professional to determine the underlying issue. They can provide a thorough evaluation and appropriate management based on your specific situation. Do not hesitate to seek help, especially since the presence of blood in your stool can indicate a need for further investigation.
