Experiencing a strong heartbeat and shortness of breath while speaking?

Hello Doctor, I have stage 1 hypertension and have been taking antihypertensive medication for about a month.
A few days ago, I suddenly felt my heart beating very strongly.
When sitting still or lying down to sleep, I have a sensation of my whole body vibrating with my heartbeat.
When I talk to someone, I feel a bit short of breath, and I experience some dizziness, leaving me feeling fatigued.
While at rest, I find myself involuntarily wanting to take deep breaths.
I measured my pulse and it was around 70 beats per minute.
During exercise, I feel completely normal with no symptoms.
My blood pressure while on medication is 127/81 and my pulse is 83.
A month ago, I had urine and blood tests, and the doctor said everything was normal.
I would like to know if this kind of situation can occur suddenly, as it has been ongoing for several days and seems to be getting longer, which is making me a bit anxious.
I have scheduled an appointment with a cardiologist, but it will take two to three days.
I wanted to ask for your opinion first.

It may be worth considering reducing the dosage by half or even stopping the medication temporarily to see if there are any changes in symptoms.
Please discuss this with your physician.

It sounds like you're experiencing a combination of symptoms that can be quite distressing, including a strong heartbeat, shortness of breath, and feelings of dizziness or fatigue. Given your history of hypertension and the fact that you are currently on medication for it, it's understandable that you would be concerned about these new symptoms.

Firstly, it's important to recognize that a strong heartbeat, or palpitations, can occur for a variety of reasons. In many cases, they are benign and can be triggered by factors such as anxiety, stress, caffeine intake, or even changes in body position. Given that you mentioned feeling this sensation while sitting or lying down, it could be related to your body's response to relaxation after being active, which can sometimes lead to heightened awareness of your heartbeat.

The sensation of your body vibrating with each heartbeat, especially when lying down, can be linked to increased awareness of your heart's activity, which can be exacerbated by anxiety or stress. It's also worth noting that when you engage in conversation or physical activity, your body naturally requires more oxygen, which can lead to feelings of breathlessness if your breathing does not keep pace with your heart rate.

Your blood pressure readings of 127/81 mmHg and pulse of 83 bpm are within normal ranges, especially considering you are on antihypertensive medication. However, the fact that you are experiencing these symptoms consistently over several days is a valid reason to seek further evaluation.
In terms of potential causes, your symptoms could be related to several factors:
1. Anxiety and Stress: Anxiety can manifest physically, leading to symptoms such as palpitations, shortness of breath, and dizziness. If you have been under increased stress or anxiety, this could be a significant contributor to your symptoms.

2. Medication Side Effects: While antihypertensive medications are generally well-tolerated, they can sometimes cause side effects that may include changes in heart rate or feelings of dizziness. It would be beneficial to discuss your current medication with your healthcare provider to ensure it is the right fit for you.

3. Dehydration or Electrolyte Imbalance: Sometimes, dehydration or imbalances in electrolytes (like potassium or magnesium) can lead to palpitations and feelings of fatigue. Ensuring you are well-hydrated and maintaining a balanced diet can help.

4. Underlying Cardiac Issues: Although your previous tests have shown normal results, it is still essential to rule out any underlying cardiac conditions, especially since you have a history of hypertension. Conditions such as arrhythmias can sometimes present with similar symptoms.

5. Respiratory Factors: If you are experiencing shortness of breath, it could also be related to respiratory issues, especially if you have a history of asthma or other lung conditions.
Given the persistence of your symptoms, it is advisable to follow up with a cardiologist as planned. They may recommend further testing, such as an echocardiogram or a Holter monitor, to assess your heart's rhythm over a longer period. Additionally, discussing your symptoms with your primary care physician can provide a more comprehensive approach to managing your health.

In the meantime, consider practicing relaxation techniques such as deep breathing exercises, mindfulness, or gentle yoga, which can help alleviate anxiety and improve your overall sense of well-being. Monitoring your caffeine intake and ensuring you are getting adequate rest can also be beneficial.

Remember, while it is easy to become anxious about symptoms, many cases are manageable with the right approach and support. Your health is important, and seeking the advice of healthcare professionals is the best course of action to address your concerns.
