I'm very worried?
Hello, doctor.
I am a high school freshman.
Recently, I noticed white spots near the urethral opening on my glans.
They are not painful or itchy, but I can feel a grainy texture when I am erect.
I can only see them clearly when I pull back the foreskin or when I am erect.
I am very scared that it might be genital warts.
I only masturbate and have never had sexual intercourse, which makes me very anxious.
I also have phimosis.
Please help me with this; I am very worried.
Thank you.

If there is no sexual activity, it is usually normal tissue.

More Info
It’s understandable to feel anxious about any changes in your body, especially in sensitive areas like the glans. Based on your description, the white spots you are observing near the urethral opening could be due to several benign conditions rather than something serious like genital warts (which is caused by the human papillomavirus, HPV).
Firstly, it’s important to note that the glans and surrounding areas can develop various types of spots or lesions that are not necessarily indicative of a sexually transmitted infection (STI). Common causes of white spots in this area include:
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans. They are a normal anatomical variant and do not require treatment.
2. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands without hair follicles. They are completely normal and common in both men and women.
3. Lichen Sclerosus: This is a skin condition that can cause white patches on the skin, including the genital area. It is usually not painful but can cause discomfort and requires medical evaluation.
4. Balanitis: This is inflammation of the glans, which can be caused by poor hygiene, infections, or irritants. It may present with redness, swelling, and sometimes white patches.
5. Genital Warts: While you mentioned a concern about warts, they typically appear as small, flesh-colored or white bumps and can be itchy or uncomfortable. However, the absence of pain or itching in your case makes this less likely.
Given that you are experiencing these symptoms without pain or itching, it is less likely to be an STI, but it is still crucial to get a proper diagnosis. Since you mentioned that you have a longer foreskin, it may also contribute to irritation or hygiene issues, which can lead to conditions like balanitis.
Here are a few recommendations:
- Maintain Good Hygiene: Ensure that you clean the area gently but thoroughly, especially under the foreskin if you have one. This can help prevent irritation and infections.
- Avoid Irritants: Be cautious with soaps, lotions, or other products that may irritate the sensitive skin in that area.
- Monitor Changes: Keep an eye on the spots. If they change in size, color, or if you develop any pain, itching, or discharge, it’s important to seek medical attention.
- Consult a Urologist: Since you are feeling anxious, it would be beneficial to consult a urologist. They can provide a thorough examination and, if necessary, perform tests to rule out any infections or other conditions.
In summary, while it’s natural to worry about new symptoms, many conditions that cause white spots on the glans are benign. However, only a healthcare professional can provide a definitive diagnosis and peace of mind. Don’t hesitate to reach out to a doctor for an evaluation; it’s the best way to address your concerns and ensure your health.
