Gum swelling and bleeding?
Hello Doctor: It has been over a year since I had my wisdom teeth extracted at the clinic, and the gums around the adjacent tooth have been persistently swollen and bleeding.
Prior to the extraction, I did not experience any discomfort in that tooth's gums.
I have undergone multiple periodontal cleanings and have taken anti-inflammatory medications and antibiotics, but they have been ineffective.
Is it possible that the sutures from the wisdom tooth extraction were not completely removed, leading to chronic inflammation of the adjacent tooth's gums? Thank you for taking the time to respond.

Chronic gum swelling and bleeding after wisdom tooth extraction can be a frustrating and concerning issue. It’s important to understand that while wisdom tooth extraction is a common procedure, it can lead to complications, especially if the healing process is not optimal.
In your case, it seems that the gum tissue around the adjacent tooth has been persistently swollen and bleeding for over a year after the extraction. This is not typical and suggests that there may be an underlying issue that needs to be addressed.
One possibility is that there may have been residual tissue or suture material left behind after the extraction. If the sutures were not fully removed or if there was any remaining granulation tissue, it could lead to chronic inflammation in the surrounding gum tissue. This inflammation can manifest as swelling, bleeding, and discomfort. It’s also possible that the extraction site did not heal properly, leading to a condition known as dry socket or alveolar osteitis, which can cause significant pain and delayed healing.
Another potential cause of your symptoms could be periodontal disease. If the adjacent tooth had pre-existing gum disease that was not evident before the extraction, the trauma from the extraction could have exacerbated the condition. Periodontal disease can lead to gum inflammation, bleeding, and even tooth mobility if not treated properly.
Additionally, it’s important to consider other factors that could contribute to gum issues, such as oral hygiene practices, smoking, or systemic health conditions like diabetes, which can impair healing and increase the risk of infections.
Given that you have already undergone multiple periodontal cleanings and have taken antibiotics without improvement, it may be time to seek a second opinion from a specialist, such as a periodontist. They can perform a thorough examination, possibly including imaging studies, to assess the health of the surrounding tissues and determine if there is any residual infection or other complications.
In the meantime, maintaining excellent oral hygiene is crucial. This includes brushing gently around the affected area, using an antibacterial mouthwash, and possibly using a water flosser to help keep the area clean. Avoiding irritants such as tobacco and ensuring a balanced diet can also support healing.
If you suspect that the sutures may be the cause of your ongoing symptoms, it’s important to have them evaluated. Sometimes, a minor surgical procedure may be necessary to remove any residual tissue or suture material that could be causing chronic irritation.
In summary, chronic gum swelling and bleeding after wisdom tooth extraction is not normal and warrants further investigation. It could be due to residual suture material, improper healing, or periodontal disease. Seeking a second opinion from a dental specialist may provide you with the answers and treatment you need to resolve this issue. Remember, your oral health is vital, and addressing these symptoms promptly can prevent further complications down the line.
